天津地区儿科呼吸道感染肺炎支原体IgM阳性率分析研究

摘　　要：目的 收集观察天津地区急性呼吸道感染的儿童中外周血检测肺炎支原体(MP)IgM抗体的临床阳性率数据,为新冠疫情之后的预防和诊治儿童肺炎支原体感染提供参考.方法 收集2022年11月-2024年1月于天津市儿童医院龙岩院区就诊治疗的241676例急性呼吸道感染患儿的临床资料,根据儿童患病的季节、性别和年龄段,比较肺炎支原体IgM阳性率.结果 2022-2023年冬季和2023年春季MP IgM阳性率明显低于2023年夏季、秋季和2023-2024年冬季MP IgM阳性率,2023年度季节MP IgM阳性率统计具有显著差异(P<0.05).女性患儿的MP IgM阳性率明显高于男性患儿,2023年度男女童MP IgM阳性率统计具有显著差异(P<0.05).年龄段6-8岁和9-11岁的患儿MP IgM阳性率明显高于其他年龄段,2023年各年龄段之间MP IgM阳性率统计具有显著差异(P<0.05).结论 天津市2023年度急性呼吸道感染患儿中MP IgM检出率为29.49%,2022-2023年冬季最低,2023年夏季感染率最高.全年都有较高的检出率,各年龄段患儿均可发生MP感染,9-11岁感染率最高,12岁以上感染率最低.2023年度女性患儿MP IgM检出率高于男性患儿.通过对2023年度18岁以下MP IgM抗体进行检测统计,可以了解到新冠疫情之后儿童急性呼吸道感染MP的情况,指导临床合理用药诊治,从而缩短患儿病程,避免病情加重.Objective To collect and observe the clinical positive rate of peripheral blood detection of Mycoplasma Pneumoniae(MP)IgM in children with acute respiratory tract infection in Tianjin,and to provide reference for the prevention and treatment of MP infection in children.Methods The clinical data of 241676 children with acute respiratory tract infection who were treated in Longyan Branch of Tianjin Children's Hospital from November 2022 to January 2024 were collected,and the positive rate of MP IgM was analyzed according to the season,gender and age of the children.Results The positive rate of MP IgM in winter of 2022-2023 and spring of 2023 was significantly lower than that in summer and autumn of 2023 and winter of 2023-2024,and the positive rate of MP IgM in season of 2023 was statistically significant(P<0.05).The positive rate of MP IgM in girls was significantly higher than that in boys,and the positive rate of MP IgM in girls and boys in 2023 was statistically significant(P<0.05).The positive rate of MP IgM in age groups of 6-8 and 9-11 years old was significantly higher than that in other age groups,and the positive rate of MP IgM in each age group in 2023 was statistically significant(P<0.05).Conclusion The detection rate of MP IgM in children with acute respiratory tract infection in Tianjin in 2023 is 29.49%,with the lowest detection rate in winter of 2022-2023 and the highest detection rate in summer of 2023.There is a high detection rate throughout the year.MP infection can occur in children of all ages,with the highest detection rate in 9-11 years old and the lowest detection rate in children over 12 years old.The detection rate of MP IgM in female children is higher than that in male children in 2023.Through the detection and statistics of MP IgM antibody in children under 18 years old in 2023,we can understand the situation of MP infection in children with acute respiratory tract infection after COVID-19,guide the clinical diagnosis and treatment of rational drug use,so as to shorten the course of di
