蒿芩清胆汤治疗老年慢性主观性耳鸣(少阳湿热型)疗效及对血清因子的影响

摘　　要：目的 探讨蒿芩清胆汤治疗老年慢性主观性耳鸣(少阳湿热型)的疗效及对血清因子的影响.方法 纳入2022年9月-2023年9月期间就诊于山东省泗水县人民医院神经内科门诊、眩晕门诊、中医科门诊的老年慢性主观性耳鸣患者60例作为研究对象,采用简单随机数表方法分为治疗组和对照组,每组30例,对照组采用草酸艾司西酞普兰片治疗,治疗组在其基础上加用蒿芩清胆汤治疗,评价治疗前后患者耳鸣致残量表(THI)评分、耳鸣严重程度评估量表(TEQ)评分的改善情况及血清因子BDNF、GABA、S100β的水平变化.结果 治疗组治疗后THI、TEQ评分明显低于对照组(P<0.05),治疗组治疗后血清BDNF、GABA水平优于对照组(P<0.05),治疗组治疗后血清S100β水平明显低于对照组(P<0.05),治疗组治疗后总有效率明显高于对照组(P<0.05).结论 蒿芩清胆汤联合草酸艾司西酞普兰片能够显著改善老年慢性主观性耳鸣的症状,其机制可能通过升高血清BDNF、GABA、降低血清S100β水平来实现的.Objective To investigate the effect of Haoqin Qingdan Decoction on senile chronic subjective tinnitus(Shaoyang Dampness-Heat Type)and its influence on serum factors.Methods 60 senile patients with chronic subjective tinnitus from September 2022 to September 2023 in neurology clinic,vertigo clinic and Traditional Chinese Medicine clinic of the People's Hospital of Sishui County,Shandong Province were divided into treatment group and control group.Each group had 30 cases.The control group was treated with escitalopram oxalate tablets,while the treatment group was treated with Haoqin Qingdan Decoction on the basis of escitalopram oxalate tablets.The improvement of THI score,TEQ score and changes of serum factors BDNF,GABA and S100βwere evaluated.Results After treatment,the scores of THI and TEQ in the treatment group were significantly lower than those in the control group(P<0.05),the levels of BDNF and GABA in the treatment group were better than those in the control group(P<0.05),the levels of S100βin the treatment group were significantly lower than those in the control group(P<0.05),and the total effective rate was significantly higher than that in the control group(P<0.05).Conclusion Haoqin Qingdan Decoction combined with escitalopram oxalate tablet can significantly improve the symptoms of senile chronic subjective tinnitus,which may be achieved by increasing serum BDNF,GABA and decreasing serum S100βlevel.
