摘  要:“椎骨错缝”是引起慢性下腰痛主要病机之一,其诱发疼痛与功能障碍是亟待解决的临床问题。推拿手法治疗慢性下腰痛疗效显著,但机制尚不明确。研究表明瞬时受体电位通道参与推拿手法镇痛机制,并且与钙离子调节关系密切。文章从瞬时受体电位敏化特性、“钙稳态”失衡与疼痛相关性等方面进行讨论,旨在阐明推拿手法可能通过“钙稳态”调节路径获效,从而进一步阐释手法治疗骨关节疾病的科学内涵,为中医非药物疗法“外治内应”潜在机制提供客观科学思路。Mild malposition of vertebra and joint is one of the main causes of chronic lower back pain,and its induced pain and functional impairment are urgent clinical issues that need to be addressed.Tuina manipulation has a significant therapeutic effect on chronic lower back pain,but the mechanism is not yet clear.Research has shown that transient receptor potential channels are involved in the mechanism of tuina analgesia and are closely related to calcium ion regulation.This article discusses the characteristics of instantaneous receptor potential sensitization,calcium homeostasis imbalance,and the correlation with pain,aiming to clarify that tuina manipulation may be effective through the'calcium homeostasis'regulation pathway,further elucidating the scientific connotation of manual therapy of bone and joint diseases,and providing objective scientific ideas for the potential mechanism of'external treatment and internal response'in traditional Chinese medicine non pharmacological therapy.
