基于“精-血-营-卫”病质理论辨治老年急性髓系白血病思路探析

摘　　要：急性髓系白血病是以老年患者多发的恶性血液系统肿瘤,由于其年龄和体质的特殊性,不适合强化疗,此时中医药对该病诊疗的介入变得尤为关键,以达成目前老年急性髓系白血病亟需实现的目标:长缓解、防复发。中医学认为,白血病病位在髓,精是生化血液的重要物质,骨髓坚固,气血皆从,营体为血,卫体为气。文章提出“精-血-营-卫”四大病质之间的平衡充盈是确保白血病细胞衰败的关键。因此总结以“精-血-营-卫”病质理论为基础,贯穿该群体诊疗过程的中医辨治方案,期望对老年白血病患者防复发、实现长生存有所助益。Acute myeloid leukemia(AML)is a malignant hematological tumor that occurs more frequently in elderly patients.Due to the special characteristics of their age and physique,it is not suitable for strong chemotherapy,and at this time,the intervention of traditional Chinese medicine(TCM)in the diagnosis and treatment of this disease has become particularly crucial to achieve the goal that AML in the elderly urgently needs to be achieved:long remission and prevention of relapse.According to TCM,the location of the disease of leukemia is located in the medulla oblongata,and essence is an important substance for blood generation and transformation.Bone marrow is strong,and qi and blood come from it;the ying is blood,and the wei is qi.This paper proposes that the balance and fullness among the four major disease quality,namely,'essence-blood-ying-wei',is the key to ensure the decay of leukemia cells.Therefore,we summarize the TCM treatment plan based on the theory of'essence-blood-ying-wei',which runs through the differentiation and treatment process of this group of patients,and hope that it will be helpful for the prevention of relapse and the realization of long-term survival of the elderly leukemia patients.
