摘 要:目的:分析2型糖尿病(T2DM)痰证、非痰证患者外周血细胞周期蛋白依赖性激酶抑制剂2A(CDKN2A)基因表达情况及CDKN2A基因、炎症相关指标(外周血炎症细胞、血清炎症因子)与痰证素积分之间的相关性。方法:根据“证素辨证”原理纳入T2DM痰证患者30例,非痰证患者24例,同时以29名健康人为对照。采集外周静脉血行生化指标、血常规检测,采用实时荧光定量PCR和ELISA法分别检测全血和血清中CDKN2A mRNA及蛋白的表达量。结果:T2DM痰证组身体质量指数(BMI)显著高于T2DM非痰证组(P<0.01)。T2DM痰证组CDKN2A蛋白表达量显著高于T2DM非痰证组(P<0.01)。T2DM非痰证组和T2DM痰证组的肿瘤坏死因子-α(TNF-α)表达量显著高于健康组(P<0.05,P<0.01),T2DM痰证组白介素(IL)-6显著高于健康组(P<0.05);TNF-α与痰证素积分呈正相关(P<0.01);T2DM痰证组中性粒细胞计数、中性粒细胞百分比、中性粒细胞/淋巴细胞比值(NLR)与痰证素积分呈正相关,淋巴细胞百分比与痰证素积分呈负相关(P<0.05,P<0.01);T2DM非痰证组单核细胞百分比与痰证素积分呈负相关(P<0.01)。结论:CDKN2A基因的高表达可能使T2DM痰证形成的风险增高,NLR、TNF-α水平可能与T2DM痰证的发生有关,提示可作为T2DM痰证诊断的辅助指标。Objective:To analyze the expression of cyclin-dependent kinase inhibitor 2A(CDKN2A)gene in peripheral blood of patients with type 2 diabetes mellitus(T2DM)with phlegm syndrome and non-phlegm syndrome,as well as CDKN2A gene and inflammation-related indicators(peripheral blood inflammatory cells,serum inflammatory factors)correlation with phlegm syndrome elements.Methods:According to the principle of syndrome differentiation,30 T2DM patients with phlegm syndrome and 24 patients with non-phlegm syndrome were included,and 29 healthy people were used as controls.Peripheral venous blood was collected for biochemical indicators and routine blood tests.Real-time quantitative PCR and ELISA were used to detect the expression of CDKN2A gene mRNA and protein in whole blood and serum,respectively.Results:Compared with T2DM non-phlegm syndrome group,T2DM phlegm syndrome group had higher BMI(P<0.01).The expression of CDKN2A protein in T2DM phlegm syndrome group was significantly higher than that in T2DM non-phlegm syndrome group(P<0.01).The expression levels of tumor necrosis factor-α(TNF-α)in T2DM non-phlegm syndrome group and T2DM phlegm syndrome group were higher than those in the healthy group(P<0.05,P<0.01),and the interleukin(IL)-6 in the T2DM phlegm syndrome group was higher than that in the healthy group(P<0.05);TNF-αwas positively correlated with phlegm syndrome(P<0.01);neutrophil count,neutrophil percentage,the neutrophils/lymphocyte ratio(NLR)in T2DM phlegm syndrome group was positively correlated with phlegm syndrome,and the percentage of lymphocytes was negatively correlated with phlegm syndrome(P<0.05,P<0.01).The percentage of monocytes in T2DM non phlegm syndrome group was negatively correlated with phlegm syndrome score(P<0.01).Conclusion:The high expression of CDKN2A gene may increase the risk of phlegm syndrome in T2DM.The levels of NLR and TNF-αmay be related to the occurrence of phlegm syndrome in T2DM,suggesting that they can be used as auxiliary indicators for the diagnosis of phlegm syndrome in T2DM.
