替米沙坦联合羟苯磺酸钙对早期糖尿病肾病患者肾脏血管的保护作用

摘  要:目的分析替米沙坦联合羟苯磺酸钙对早期糖尿病肾病患者肾脏血管的保护作用。方法回顾性分析2021年10月至2024年10月河南大学附属郑州颐和医院收治的100例早期糖尿病肾病患者的临床资料,将采取替米沙坦治疗的50例患者作为对照组,将采取替米沙坦联合羟苯磺酸钙治疗的50例患者作为观察组。对比两组疗效、血糖指标[空腹血糖(FPG)、餐后2 h血糖(2hPG)、糖化血红蛋白(HbA1C)]水平、肾功能[血肌酐(SCr)、尿素氮(BUN)、β2-微球蛋白(β2-MG)]水平、炎性因子[白细胞介素6(IL-6)、肿瘤坏死因子-α(TNF-α)]水平、血管内皮功能[内皮素-1(ET-1)、一氧化氮(NO)、血管内皮生长因子(VEGF)]水平、不良反应。结果观察组治疗效率[98.00%(49/50)]较对照组[80.00%(40/50)]高(P<0.05);治疗后,观察组FPG[(7.02±0.32)mmol/L]、2hPG[(9.76±0.43)mmol/L]、HbA1C[(6.05±0.47)%]、SCr[(66.52±5.34)μmol/L]、BUN[(4.57±0.36)mmol/L]、β2-MG[(0.25±0.03)μg/L]、IL-6[(80.42±7.16)pg/mL]、TNF-α[(8.75±0.85)pg/mL]、ET-1[(63.74±4.21)ng/L]水平较对照组[(9.86±0.47)mmol/L、(11.83±0.56)mmol/L、(8.47±0.58)%、(72.68±5.67)μmol/L、(5.92±0.43)mmol/L、(0.39±0.04)μg/L、(92.57±7.62)pg/mL、(10.46±0.92)pg/mL、(68.52±4.65)ng/L]低,NO[(69.25±5.37)μmol/L]、VEGF[(134.82±10.47)pg/mL]水平较对照组[(61.43±5.15)μmol/L、(127.53±9.52)pg/mL]高(P<0.05);两组不良反应对比无显著差异(P>0.05)。结论替米沙坦联合羟苯磺酸钙应用可降低血糖水平,改善肾功能,减轻炎症症状,保护血管内皮功能,且安全性较佳。Objective To analyze the protective effect of telmisartan combined with calcium hydroxybenzenesulfonate on renal vessels in patients with early diabetes nephropathy.Methods From October 2021 to October 2024,the clinical information of 100 patients with early diabetes nephropathy in Zhengzhou Yihe Hospital Affiliated to Henan University were retrospectively analyzed.50 patients treated with telmisartan were taken as the control group,and 50 patients treated with telmisartan combined with calcium dobesilate were taken as the observation group.The efficacy,blood glucose indicators[fasting blood glucose(FPG),2-hour postprandial blood glucose(2hPG),glycated hemoglobin(HbA1C)]levels,renal function[serum creatinine(SCr),blood urea nitrogen(BUN),β2-microglobulin(β2-MG)]levels,inflammatory factors[interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)]levels,vascular endothelial function[endothelin-1(ET-1),nitric oxide(NO),vascular endothelial growth factor(VEGF)]levels,and adverse reactions of two groups were compared.Results The treatment efficiency of the observation group[98.00%(49/50)]was higher than that of the control group[80.00%(40/50)](P<0.05).After treatment,FPG[(7.02±0.32)mmol/L],2hPG[(9.76±0.43)mmol/L],HbA1C[(6.05±0.47)%],SCr[(66.52±5.34)μmol/L],BUN[(4.57±0.36)mmol/L],β2-MG[(0.25±0.03)μg/L],IL-6[(80.42±7.16)pg/mL],TNF-α[(8.75±0.85)pg/mL],and ET-1[(63.74±4.21)ng/L]in the observation group were lower than those in the control group[(9.86±0.47)mmol/L,(11.83±0.56)mmol/L,(8.47±0.58)%,(72.68±5.67)μmol/L,(5.92±0.43)mmol/L,(0.39±0.04)μg/L,(92.57±7.62)pg/mL,(10.46±0.92)pg/mL,68.52±4.65)ng/L],and NO[(69.25±5.37)μmol/L]and VEGF[(134.82±10.47)pg/mL]levels were higher than those in the control group[(61.43±5.15)μmol/L,(127.53±9.52)pg/mL](P<0.05);There was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion The combination of telmisartan and calcium hydroxybenzenesulfonate can lower blood glucose levels,improve renal function,alleviate inflammatory symptoms
