玻璃酸钠关节腔注射配合中医推髌针刺手法治疗中老年髌股关节病效果观察

摘　　要：目的 探讨玻璃酸钠关节腔注射配合中医推髌针刺手法治疗中老年髌股关节病的效果。方法 选取2023年6月至2024年12月在赣州市中医院就诊的60例髌股关节病患者,根据患者治疗意愿及治疗方式分两组,每组30例。对照组采取玻璃酸钠关节腔注射,观察组在此基础上加用中医推髌针刺手法。治疗5周后对比两组疼痛程度、膝关节恢复情况、膝关节功能。结果 治疗前,两组视觉模拟评分(VAS)、国际膝关节评估表(IKDC)、美国特种外科医院膝关节评分系统(HSS)分值比较,差异无统计学意义(P>0.05);治疗后,观察组VAS为(1.26±0.24)分,低于对照组的(2.19±0.37)分,差异有统计学意义(t=11.550,P<0.001);观察组IKDC为(80.30±3.13)分、HSS为(81.65±3.01)分,高于对照组的(75.58±3.02)分、(74.45±3.54)分,差异有统计学意义(t=5.944、8.487,P均<0.001)。结论 玻璃酸钠关节腔注射配合中医推髌针刺手法治疗中老年髌股关节病,有助于减轻疼痛感,改善膝关节功能,加速患者恢复。Objective To explore the effect of sodium hyaluronate intra-articular injection combined with traditional Chinese medicine patellar massage and acupuncture in the treatment of patellofemoral arthropathy in middle-aged and elderly patients.Methods Sixty patients with patellofemoral arthropathy who admitted to Ganzhou Hospital of Traditional Chinese Medicine from June 2023 to December 2024 were taken as the subjects.All cases were assigned to two groups based on their treatment willingness and treatment methods,with 30 cases in each group.The patients of control group adopted intra-articular injection of sodium hyalur-onate,and on this basis,the patients of observation group received traditional Chinese medicine patellar massage and acupuncture.The comparisons were made on the pain level,knee joint recovery,and knee joint function in both groups after 5 weeks of treat-ment.Results The Visual Analogue Scale(VAS),International Knee Documentation Committee(IKDC)and the Hospital for Spe-cial Surgery(HSS)scores of both groups before treatment were compared,which showed no statistical difference(P>0.05);The VAS score of observation group[(1.26±0.24)points]was lower compared with the control group[(2.19±0.37)points]after treatment(t=11.550,P<0.001);The IKDC and HSS scores of the observation group were(80.30±3.13)points and(81.65±3.01)points,respectively,which were significantly higher than(75.58±3.02)points and(74.45±3.54)points of the control group(t=5.944 and 8.487,P<0.001).Conclusion The combination of sodium hyaluronate intra-articular injection and tradition-al Chinese medicine patellar massage and acupuncture for the treatment of patellofemoral arthropathy in middle-aged and elderly pa-tients can help to alleviate pain,improve knee joint function,and accelerate patients'recovery.
