摘　　要：目的 探讨知柏地黄丸联合醋酸曲普瑞林、重组人生长激素(rhGH)对大骨龄中枢性性早熟(CPP)女童性激素及生长发育的影响。方法 前瞻性选取2023年6月至2024年6月在赣州市妇幼保健院就诊的大骨龄CPP女童100例,根据随机数字表法分为对照组和实验组,每组50例。对照组患儿使用醋酸曲普瑞林与rhGH治疗,实验组患儿使用知柏地黄丸联合醋酸曲普瑞林、rhGH治疗。比较两组的临床疗效、性激素指标[黄体生成素(LH)、卵泡刺激素(FSH)、雌二醇(E2)]、骨代谢指标[Ⅰ型胶原羧基端肽β特殊序列(β-CTX)、N端骨钙素(N-MID)]以及生长发育指标[身高、体质量指数(BMI)、年平均生长速度(ΔGV)、骨龄差值(ΔBA)、子宫体积、卵巢体积]及不良反应发生率。结果 治疗后,患儿的E2、LH、FSH、β-CTX、N-MID水平以及子宫体积、卵巢体积、ΔBA均减少,而身高、BMI、ΔGV均升高,与治疗前相比有显著差异(P<0.05)。实验组的E2、LH、FSH、β-CTX、N-MID水平以及子宫体积、卵巢体积、ΔBA均低于对照组,而身高、BMI、ΔGV均高于对照组(P<0.05)。实验组临床总有效率高于对照组(P=0.017)。两组不良反应发生率无差异(P=0.712)。结论 知柏地黄丸联合醋酸曲普瑞林、rhGH治疗可以降低CPP患儿的性激素水平,改善生长发育情况,疗效更佳。Objective To analyze the effects of Zhibai Dihuang pill combined with triptorelin acetate and recombinant human growth hormone(rhGH) treatment on sex hormones and growth and development in girls with central precocious puberty(CPP) of large bone age. Methods A total of 100 girls with CPP of large bone age who were treated in the Ganzhou Maternal and Child Health Care Hospital from June 2023 to June 2024 were prospectively selected and divided into control group and experimental group according to the random number table method, with 50 cases in each group. The control group of children was treated with triptorelin acetate and rhGH, while the experimental group of children was treated with Zhibai Dihuang pill combined with triptorelin acetate and rhGH. The clinical efficacy, sex hormone indexes [luteinizing hormone(LH), follicle stimulating hormone(FSH), estradiol(E_2)], bone metabolism indexes [β-C-terminal telopeptide of type Ⅰ collagen(β-CTX), N-terminal osteocalcin(N-MID)], growth and development indexes [height, body mass index(BMI), average annual growth rate(ΔGV), bone age difference(ΔBA), uterine volume, ovarian volume] and the incidence of adverse reactions were compared between the two groups. Results After treatment, the levels of E_2, LH, FSH, β-CTX, N-MID, uterine volume, ovarian volume, and Δ BA in the children decreased, while their height, BMI, and Δ GV increased significantly compared to before treatment(P<0.05). The levels of E_2, LH, FSH, β-CTX, N-MID, uterine volume, ovarian volume, and ΔBA in the experimental group were lower than those in the control group, while height, BMI, and ΔGV were higher than those in the control group(P<0.05). The total clinical effective rate of the experimental group was higher than that of the control group(P=0.017). There was no significant difference in the incidence of adverse reactions between the two groups(P=0.712). Conclusion The combination of Zhibai Dihuang pill with triptorelin acetate and rhGH treatment can reduce the levels of sex hormones
