中药治疗耐药性脊柱结核的研究进展

摘　　要：脊柱结核的发病和转归与结核分枝杆菌的生长和抑制、宿主的免疫状态密切相关,现有抗结核一线药物和二线药物存在交叉耐药,且不良反应重,免疫调节作用差,使耐药结核病及免疫功能低下的结核病患者在治疗时遇到困难。中药在减轻结核病症状、联合西药治疗结核病,以及提高患者对结核分枝杆菌的免疫水平方面已有很多研究。本文对中药小分子化学物质及中药提取物对结核病治疗研究进展进行综述,旨在为中药辅助治疗耐药性脊柱结核提供科学依据,为临床医师提供更多的中西医结合治疗耐药性脊柱结核方面的思路与策略。The onset and prognosis of spinal tuberculosis are closely related to the growth and inhibition of Mycobacterium tuberculosis and the immune status of the host.First-line and second-line anti-tuberculosis drugs currently exhibit cross-resistance and severe adverse effects,with poor immune modulation.This poses challenges in the treatment of drug-resistant and immunocompromised tuberculosis patients.There have been numerous studies on the use of traditional Chinese medicine in alleviating tuberculosis symptoms,combining therapy with western medicine,and improving the immune response against Mycobacterium tuberculosis.This article provides a comprehensive review of the research progress in the use of small-molecule chemical substances and extracts of traditional Chinese medicine for the treatment of tuberculosis.Its aim is to provide scientific evidence for the adjunctive treatment of drug-resistant spinal tuberculosis with traditional Chinese medicine and offer more insights and strategies for clinicians in integrating traditional Chinese and Western medicine in the treatment of drug-resistant spinal tuberculosis.
