内窥镜下鼻腔泪囊吻合术联合泪小管置管术治疗鼻泪管阻塞的效果及对泪液分泌的影响

摘　　要：目的探讨内窥镜下鼻腔泪囊吻合术治疗鼻泪管阻塞中联合泪小管置管术的效果及对泪液分泌的影响。方法选取重庆市丰都县人民医院2022年5月至2024年5月收治的96例鼻泪管阻塞患者,以数字表法随机分为对照组和观察组,每组各48例,分别予以内窥镜下鼻腔泪囊吻合术和内窥镜下鼻腔泪囊吻合术联合泪小管置管术,对比分析2组疗效、泪液分泌情况、血清炎症水平、并发症发生率及生活质量。结果组间疗效相比,观察组更高(P<0.05)。术后3个月,与对照组相比,观察组泪液弯液面水平更低,泪液分泌量更(P<0.05)。术后3个月,与对照组肿瘤坏死因子α(TNF-α)、超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)相比,观察组更低(P<0.05)。围术期并发症比较,观察组2.08%与对照组8.33%的发生率相比无统计学差异(P>0.05)。术后3个月,与对照组相比,观察组眼表症状评分更低,健康调查简表(SF-36)评分更高(P<0.05)。结论内窥镜下鼻腔泪囊吻合术联合泪小管置管术治疗鼻泪管阻塞可提高疗效,改善患者泪液分泌,减轻炎症反应,提高生活质量,且并发症少。Objective To investigate the effect of endoscopic nasodacryocystostomy in the treatment of nasolacrimal duct obstruction combined with lacrimal duct catheterization and its influence on tear secretion.Methods From May 2022 to May 2024,96 patients with nasolacrimal duct obstruction admitted to our hospital were randomly divided into 2 groups by numerical table method.48 cases in the control group and 48 cases in the observation group were given endoscopic nasolacrimal cystostomy and endoscopic nasolacrimal cystostomy combined with lacrimal duct catheterization,respectively.The efficacy,tear secretion,serum inflammation,complication rate,and quality of life of the two groups were compared and analyzed.Results Comparison of efficacy between groups,the observation group was higher(P<0.05).Three months after surgery,compared with the control group,the level of tear curvature was lower and the volume of tear secretion was higher in the observation group(P<0.05).Three months after surgery,tumor necrosis factor-α(TNF-α),hypersensitive C-reactive protein(hs-CRP),and interleukin-6(IL-6)were lower in the observation group than in the control group(P<0.05).There was no significant difference in the incidence of perioperative complications between the observation group(2.08%)and the control group(8.33%)(P>0.05).Three months after surgery,the ocular surface symptom score was lower and the SF-36 score was higher in the observation group compared with the control group(P<0.05).Conclusion Endoscopic nasodacryocystostomy combined with lacrimal duct catheterization in the treatment of nasolacrimal duct obstruction can improve the curative effect,improve the secretion of tears,reduce the inflammatory response,improve the quality of life,and reduce complications.
