摘  要:目的研究胸腺肽配合异丙托溴铵雾化吸入治疗慢性阻塞性肺疾病(COPD)合并重症肺炎疗效及对外周血正五聚体蛋白-3(PYX-3)、人嗜酸粒细胞趋化蛋白-2(eotaxin-2)、可溶性髓样细胞表达的激发受体-1(sTREM-1)水平的影响。方法将江苏省人民医院溧阳分院呼吸内科于2020年1月至2023年6月收治的200例COPD合并重症肺炎患者纳为研究对象,按单双号法将所有患者分为常规组(单号,n=100)与结合组(双号,n=100),常规组患者给予异丙托溴铵雾化吸入剂治疗,剂量为2 mL/次,2次/d;结合组患者在此基础上给予胸腺肽注射治疗,剂量为60 mg/次,1次/d,2组均持续治疗10 d。对比2组患者临床疗效、症状改善时间、治疗前后动脉血氧分压、炎症指标。结果结合组患者治疗有效率(99.0%)高于常规组(94.0%)(P<0.05);结合组症状改善时间及住院时间均短于常规组(P<0.05);治疗后,2组PaO_(2)与氧合指数升高(P<0.05),PaCO_(2)下降(P<0.05),结合组治疗后的PaO_(2)、氧合指数均高于常规组(P<0.05),PaCO_(2)明显低于常规组(P<0.05);治疗后,2组患者PYX-3、eotaxin-2、sTREM-1及CRP水平均下降(P<0.05),且结合组低于常规组(P<0.05)。结论胸腺肽配合异丙托溴铵雾化吸入治疗可明显改善COPD合并重症肺炎患者血压分压,降低炎症反应,临床值得推广应用。Objective To explore the efficacy of thymosin combined with ipratropium bromide aerosol inhalation in the treatment of chronic obstructive pulmonary disease(COPD)with severe pneumonia.Methods 200 patients with COPD and severe pneumonia admitted to the department of respiratory medicine of our hospital from January 2020 to June 2023 were enrolled in the study.All patients were divided into a routine group(odd number,n=100)and a combined group(even number,n=100)according to the odd-even number method.The patients in the routine group were treated with ipratropium bromide aerosol inhalation at a dose of 2 mL/time for twice a day,and on this basis,the patients in the combined group were treated with thymosin injection at a dose of 60 mg/time for once a day.Both groups were treated continuously for 10 days.The clinical efficacy,symptom improvement time,arterial oxygen partial pressure,and inflammatory indicators were compared between the two groups.Results The effective rate of treatment of patients in the combined group was significantly higher than that in the routine group(P<0.05).The symptom improvement time and hospital stay were significantly shorter in the combined group than those in the routine group(P<0.05).After treatment,PaO_(2) and oxygenation index in the two groups were significantly increased(P<0.05),while PaCO_(2) was significantly decreased(P<0.05).PaO_(2) and oxygenation index in the combined group after treatment were significantly higher(P<0.05),while PaCO_(2) was significantly lower compared to the routine group(P<0.05).After treatment,the levels of PYX-3,eotaxin-2,sTREM-1,and CRP were significantly reduced in the two groups(P<0.05),and the combined group had significantly lower levels(P<0.05).Conclusion Thymosin combined with ipratropium bromide aerosol inhalation can significantly improve the oxygen partial pressure and reduce the inflammatory response in patients with COPD complicated with severe pneumonia.
