摘 要:目的探讨外周血辅助性T细胞17(Th17)相关细胞因子、CD4^(+)调节性T淋巴细胞(Treg)在糖尿病周围神经病变(DPN)患者中的表达及其相关性分析。方法选取2023年1—12月南昌市第三医院收治的2型糖尿病(T2DM)患者78例,将符合DPN诊断的患者作为DPN组(n=42),未合并DPN患者作为非DPN组(n=36)。采用流式荧光法检测人Th17亚群相关细胞因子,流式细胞术检测Treg细胞,并进行相关性分析。结果DPN组糖化血红收白(HbA1c)、白细胞介素-6(IL-6)及白细胞介素-17A(IL-17A)水平显著高于非DPN组(P<0.05),DPN组Treg水平显著低于非DPN组(P<0.05),IL-17A、Treg是DPN发病的独立危险因素,在DPN组中,IL-17A与IL-6呈正相关(r=0.356,P=0.017),IL-17A与Treg呈负相关(r=-0.438,P=0.004)。结论DPN患者出现Th17细胞因子及CD4^(+)调节性T淋巴细胞失衡,IL-17A、Treg可能参与DPN发展的进程。Objective To investigate the expression levels of peripheral blood helper T cell 17(Th17)-related cytokines and CD4^(+)regulatory T lymphocytes(Treg)and analyzing their correlation with patients with diabetic peripheral neuropathy(DPN).Methods 78 patients with T2DM diagnosed at Nanchang People’s Hospital from January to December 2023 were enrolled.Patients meeting diagnostic criteria for DPN were classified as the DPN group(n=42),while those without DPN constituted the NDPN group(n=36).Th17 subsetrelated cytokines were quantified using flow fluorescence assay,and Treg cells were analyzed via flow cytometry.Statistical correlations were further evaluated.Results The DPN group exhibited significantly higher levels of HbA1c,IL-6,and IL-17A compared to the NDPN group(P<0.05),whereas Treg levels were significantly reduced(P<0.05).IL-17A and Treg were identified as independent risk factors for DPN pathogenesis.Within the DPN group,IL-17A demonstrated a positive correlation with IL-6(r=0.356,P=0.017)and a negative correlation with Treg(r=−0.438,P=0.004).Conclusion Patients with DPN display an imbalance in Th17-related cytokines and CD4^(+)regulatory T Lymphocytes.IL-17A and Treg may be involved in the pathogenic mechanisms underlying DPN progression.
