点面线式耳穴按摩联合探针刺激对慢性意识障碍患者肠道功能的影响

摘　　要：目的 在综合康复治疗基础上,探讨点面线式耳穴按摩联合探针刺激对慢性意识障碍(pDOC)患者肠道功能的调节效果。方法 将60例pDOC肠功能紊乱患者按随机数字表法分为观察组(30例)与对照组(30例)。对照组接受综合康复治疗,观察组在对照组基础上增加点面线式耳穴按摩和探针刺激,治疗2周。分别在干预前、干预后第2周比较2组Bristol粪便性状量表(BSFS)评分、Hart and Dobb腹泻评分、意识状态评分[格拉斯哥评分表(GCS)和改良国际昏迷恢复量表(CRS-R)]及便秘和腹泻发生率。结果 干预后第2周,观察组BSFS、GCS、CRS-R评分均高于对照组,便秘、腹泻发生率低于对照组(P <0.05);2组Hart and Dobb腹泻评分比较差异无统计学意义(P> 0.05)。结论 在综合康复治疗基础上,增加点面线式耳穴按摩和探针刺激可调节pDOC患者肠道功能,减少肠道功能障碍的发生,改善患者的营养状况,促进其意识恢复。Objective To investigate the effects of point-surface-line auricular point massage and probe stimulation on the regulation of intestinal function in patients with prolonged disorders of consciousness(pDOC)on the basis of comprehensive rehabilitation therapy.Methods 60 patients with pDOC intestinal dysfunction were divided into an observation group(30 cases)and a control group(30 cases)using a random number table.The control group received comprehensive rehabilitation therapy,whereas the observation group further received point-surface-line auricular acupoint massage and probe stimulation,both treated for 2 weeks.The Bristol Stool Form Scale(BSFS)score,Hart and Dobb diarrhea score,consciousness state scores[Glasgow Coma Scale(GCS)and the Revised Coma Recovery Scale(CRS-R)],and incidence of constipation and diarrhea were compared between the 2 groups before intervention and at 2 weeks after intervention.Results At 2 weeks after intervention,the observation group had higher BSFS,GCS,and CRS-R scores compared to the control group,while the incidence of constipation and diarrhea was lower in the observation group(P<0.05).However,there was no statistically significant difference in the Hart and Dobb diarrhea scores between the 2 groups(P>0.05).Conclusion Based on comprehensive rehabilitation therapy,the addition of point-surface-line auricular acupoint massage and probe stimulation can regulate intestinal function,reduce intestinal dysfunction,and improve nutritional status in pDOC patients,contributing to their consciousness recovery.
