冰冻切片病理诊断在甲状腺微小乳头状癌术中应用体会

摘　　要：目的:探讨冰冻切片病理诊断在甲状腺微小乳头状癌术中的应用价值。方法:选取菏泽医学专科学校附属医院在2020年1月—2023年12月收治的100例疑似甲状腺微小乳头状癌患者。患者均实施术前细针穿刺活检诊断、术中冰冻切片病理诊断、术后石蜡切片病理诊断,以后者诊断结果为金标准,比较前两者诊断甲状腺微小乳头状癌及淋巴结转移的效能。结果:在100例疑似甲状腺微小乳头状癌患者中,有69例患者经金标准确诊患有甲状腺微小乳头状癌;其中,发生淋巴结转移24例,未发生45例。术中冰冻切片病理诊断甲状腺微小乳头状癌及淋巴结转移的各项效能值均高于术前细针穿刺活检诊断(P<0.05)。结论:术中冰冻切片病理在甲状腺微小乳头状癌术中诊断中的应用价值高,与术前细针穿刺活检诊断相比,可减少漏诊、误诊的情况,并能对该病淋巴结转移进行准确诊断。Objective:To explore the application value of frozen section pathological diagnosis in surgery for papillary thyroid microcarcinoma.Methods:100 suspected patients with papillary thyroid carcinoma admitted to the Affiliated Hospital of Heze Medical College from January 2020 to December 2023 were selected as the study subjects.All patients underwent preoperative fine needle aspiration biopsy diagnosis,intraoperative frozen section pathological diagnosis,and postoperative paraffin section pathological diagnosis.The results of the postoperative paraffin section pathological diagnosis were used as the gold standard to compare the efficacy of the first two methods in diagnosing papillary thyroid microcarcinoma and lymph node metastasis.Results:Among the100 suspected patients,69 were diagnosed with papillary thyroid carcinoma by the gold standard.Among them,24 patients developed lymph node metastasis,and the remaining 45 patients did not develop lymph node metastasis.The efficacy values of intraoperative frozen section pathology diagnosis for thyroid papillary carcinoma and lymph node metastasis were higher than those of preoperative fine needle aspiration biopsy diagnosis(P<0.05).Conclusion:Frozen section pathology has high diagnostic value in the intraoperative diagnosis of papillary thyroid carcinoma.Compared with preoperative fine needle biopsy diagnosis,it can reduce missed diagnosis and misdiagnosis,and accurately diagnose lymph node metastasis of the disease.
