疏风清热法内外结合治疗儿童特应性皮炎的效果分析

摘　　要：目的:探讨疏风清热法内外结合治疗对儿童特应性皮炎的治疗效果。方法:以随机数字表法将滁州市中西医结合医院2020年6月—2023年6月收治的80例特应性皮炎患儿分为两组,每组各40例。对照组进行常规西医治疗,试验组在进行常规西医治疗的基础上加用中医疏风清热法内外结合治疗。对比两组的临床疗效、1年复发率、病情严重程度、相关因子的水平及近远期生活质量。结果:试验组总有效率高于对照组,1年复发率低于对照组(P<0.05)。相比治疗前,治疗后两组的特应性皮炎评分量表各项评分及总分均降低,且试验组低于对照组(P<0.05)。相比治疗前,治疗后两组的干扰素-γ(IFN-γ)均升高,且试验组高于对照组(P<0.05)。相比治疗前,治疗后两组的白细胞介素-4(IL-4)、免疫球蛋白E(Ig E)均降低,且试验组低于对照组(P<0.05)。治疗后4周和治疗后1年,与对照组相比,试验组的皮肤病生活质量指数(DLQI)评分更低(P<0.05)。试验组治疗后4周和治疗后1年的DLQI评分均低于治疗前(P<0.05)。对照组治疗后1年的DLQI评分高于治疗后4周(P<0.05)。试验组的不良反应发生率低于对照组(P<0.05)。结论:疏风清热法内外结合治疗儿童特应性皮炎的疗效显著,可降低其远期复发率,改善其症状严重程度、皮损严重程度及病变范围,同时可缓解其机体的炎症反应程度,调节免疫功能,降低不良反应发生率,提高其近远期生活质量。Objective:To explore the therapeutic effect of the combined internal and external treatment of wind clearing and heat clearing methods on atopic dermatitis in children.Methods:80 children with atopic dermatitis admitted to Chuzhou Integrated Traditional Chinese and Western Medicine Hospital from June 2020 to June 2023 were selected and divided into a control group and an experimental group by a random number table method,with 40 cases in each group.The control group received conventional Western medicine treatment,while the experimental group received a combination of internal and external treatment with wind clearing and heat clearing methods in addition to conventional Western medicine treatment.The clinical efficacy,1-year recurrence rate,severity of disease,level of related factors and shortterm and long-term quality of life were compared between the two groups.Results:The total effective rate of the experimental group was higher than that of the control group,and the one-year recurrence rate was lower than that of the control group(P<0.05).Compared with before treatment,after treatment,the SCORAD scores and total scores of both groups decreased,and the experimental group was lower than the control group(P<0.05).Secondly,both groups showed an increase in IFN-γlevels,with the experimental group being higher than the control group(P<0.05).Finally,both groups showed a decrease in IL-4 and IgE levels,with the experimental group being lower than the control group(P<0.05).Compared with the control group,the experimental group had lower DLQI scores at 4 weeks and 1 year after treatment(P<0.05),and its DLQI scores were lower than before treatment(P<0.05).The DLQI score of the control group was higher at 1 year after treatment than at 4 weeks after treatment(P<0.05).The incidence of adverse reactions in the experimental group was lower than that in the control group(P<0.05).Conclusion:The combination of internal and external treatment with the method of dispelling wind and clearing heat has significant therapeutic ef
