人工气道管理小组护理干预对重症肺炎伴呼吸衰竭患者肺部微生态以及血气分析指标的影响

摘  要:目的:探讨人工气道管理小组护理干预对重症肺炎伴呼吸衰竭患者肺部微生态以及血气分析指标的影响。方法:选取2021年6月至2023年12月在周口市中心医院治疗的重症肺炎伴呼吸衰竭患者136例作为研究对象,分为对照组、观察组,每组68例患者。对照组行常规护理干预,观察组联合人工气道管理小组护理干预。对比两组血气分析指标、肺部微生态、炎性因子、肺功能以及不良反应发生率。结果:护理后,观察组动脉血二氧化碳分压(PaCO_(2))水平明显比对照组低,动脉血氧分压(PaO_(2))、血氧饱和度(SaO_(2))水平明显比对照组高,差异显著(P<0.05);观察组的革兰氏阴性杆菌、革兰氏阳性球菌以及真菌阳性检出率均明显低于对照组,差异显著(P<0.05);观察组C反应蛋白(CRP)、降钙素原(PCT)及白细胞介素-6(IL-6)水平均明显低于对照组,差异显著(P<0.05);观察组用力呼气量(FVC)、第1秒用力呼气量(FEV1)以及FEV1/FVC水平均明显高于对照组,差异显著(P<0.05);观察组的呼吸机相关性肺炎(VAP)发生率、再插管率以及气道黏膜出血率均明显低于对照组,差异显著(P<0.05)。结论:将人工气道管理小组护理干预应用于重症肺炎伴呼吸衰竭患者护理中,可以明显改善其血气指标以及肺部微生态,值得临床推广应用。Objective:To explore the effects of nursing interventions by artificial airway management teams on lung microbiota and blood gas analysis indexes in patients with severe pneumonia and respiratory failure.Methods:136 patients with severe pneumonia and respiratory failure treated at Zhoukou Central Hospital from June 2021 to December 2023 were selected and divided into a control group and an observation group,with 68 patients in each group.The control group received routine nursing intervention,while the observation group received nursing intervention in combination with artificial airway management group.Blood gas analysis indexes,lung microbiota,inflammatory factors,lung function and incidence of adverse reactions were compared between the two groups.Results:After nursing,compared with the control group,firstly,the level of arterial partial pressure of carbon dioxide(PaCO_(2))in the observation group was significantly lower,while the level of arterial partial pressure of oxygen(PaO_(2))and blood oxygen saturation(SaO_(2))were significantly higher.Secondly,the detection rates of Gram negative bacilli,Gram positive cocci,and fungi in the observation group were significantly lower.Then,the levels of C-reactive protein(CRP),procalcitonin(PCT),and interleukin-6(IL-6)in the observation group were significantly lower.Furthermore,the observation group showed significantly higher levels of forced vital capacity(FVC),force expiratory volume in 1 second(FEV1),and FEV1/FVC ratio.Finally,the incidence of ventilator associated pneumonia(VAP),the rate of re-intubation and the rate of airway mucosal bleeding were significantly lower in the observation group.The above comparison showed significant differences(P<0.05).Conclusion:The application of nursing intervention by artificial airway management group in the nursing of patients with severe pneumonia and respiratory failure can significantly improve their blood gas indexes and lung microbiota,which is worthy of clinical promotion and application.
