复方番石榴制剂治疗2型糖尿病的经济学评价

摘　　要：目的比较复方番石榴制剂与西格列汀分别联合二甲双胍片治疗2型糖尿病(T2DM)的成本-效用。方法以支付者为研究角度,采用决策树模型对复方番石榴制剂与西格列汀分别联合二甲双胍片治疗T2DM进行成本-效用分析,并进行敏感性分析。结果与西格列汀+二甲双胍组相比,复方番石榴制剂与二甲双胍联合治疗组的增量成本-效果比(ICER)值为231211.57元/QALY,略低于3倍我国2023年人均国内生产总值(GDP,89358元),具有成本-效用优势。单因素敏感性分析结果显示,复方番石榴制剂联合二甲双胍组的健康效用值等变量对经济性分析的结果影响较大。概率敏感性分析结果则提示成本-效用分析结果较稳健。结论在二甲双胍基础上加用复方番石榴制剂治疗T2DM可取得较好的健康产出,具有长期药物经济性,为中药复方番石榴制剂尽快广泛进入市场提供了经济学评价及成本核算依据。Objective To compare the cost-effectiveness of Fufang Fanshiliu Decoction plus metformin and sitagliptin plus metformin separately in the treatment of type 2 diabetes mellitus(T2DM).Methods From the perspective of payer,decision tree model was used to analyze the cost-effectiveness and sensitivity of Fufang Fanshiliu Decoction and sitagliptin combined with metformin tablets in the treatment of T2DM.Results Fufang Fanshiliu Decoction plus metformin had a higher expected QALYs than sitagliptin plus metformin but a higher per-patient cost.Compared with sitagliptin plus metformin,Fufang Fanshiliu Decoction produced an incremental cost-effectiveness ratio of CNU231211.57/QALY,which is below the WTP threshold.The cost-effectiveness analysis ratio of Fufang Fanshiliu Decoction was less than three times of China's per capita GDP in 2023(yuan).The results of sensitivity analysis showed that the health utility value of combined with metformin tablets and other variables had a great influence on the results of cost-effectiveness analysis.The results of probabilistic analysis suggest that the basic analysis results of this study are robust.Conclusion The addition of Fufang Fanshiliu Decoction on the basis of metformin in the treatment of T2DM can achieve better health output,and has long-term drug economy,which provides economic evaluation and cost accounting basis for the rapid and extensive entry of Fufang Fanshiliu Decoction into the market.
