摘  要:目的探讨丹参多酚酸盐对慢性心力衰竭患者miR与Fas及其配体(FasL)的影响。方法选取2021年6月至2023年7月苏州大学附属第二医院收治的100例慢性心力衰竭患者,根据治疗方案不同分为对照组与观察组,每组50例。对照组予以常规慢性心力衰竭治疗,观察组则在对照组常规心力衰竭治疗措施基础上采用丹参多酚酸盐治疗。比较两组治疗总有效率、不良反应发生率、治疗前后心功能指标[左室射血分数(LVEF)、左室收缩末期内径(LVESD)、左室舒张末期内径(LVEDD)]及6MWT、miR-21及miR-199a、Fas及FasL。结果观察组治疗总有效率为96.00%,显著高于对照组的76.00%(P<0.05),两组不良反应发生率无显著差异(P>0.05);治疗前两组心功能指标、6MWT、miR-21、miR-199a、Fas及FasL差异无统计学意义(P>0.05),治疗2周及4周后观察组LVEF及6MWT显著高于对照组(P<0.05),LVESD、LVEDD、miR-21、miR-199a、Fas及FasL显著低于对照组(P<0.05)。结论丹参多酚酸盐用于治疗慢性心力衰竭临床效果显著,可有效提高患者心功能,miR-21及miR-199a、Fas及FasL的表达改善效果更好,且安全性较高。Objective To explore the effects of salvianolate on miR and Fas and its ligands(FasL)in patients with chronic heart failure.Methods A total of 100 patients with chronic heart failure admitted to the Second Affiliated Hospital of Soochow University from June 2021 to July 2023 were selected and divided into control group and observation group according to different treatment plans,with 50 cases in each group.The control group was treated with conventional chronic heart failure,and the observation group was treated with salvianolate on the basis of conventional heart failure in the control group.The total effective rate,incidence of adverse reactions,cardiac function indexes left ventricular ejection fraction(LVEF),left ventricular end-systolic diameter(LVESD),left ventricular end-diastolic diameter(LVEDD)and 6MWT,miR-21 and miR-199a,Fas and FasL before and after treatment were compared between the two groups.Results The total effective rate of the observation group was 96.00%,which was significantly higher than that of the control group(76.00%)(P<0.05),and there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).There was no significant difference in cardiac function index,6MWT,miR-21,miR-199a,Fas and FasL between the two groups before treatment(P>0.05).LVEF and 6MWT in the observation group were significantly higher than those in the control group after 2 and 4 weeks of treatment(P<0.05),LVESD,LVEDD,miR-21,miR-199a,Fas and FasL were significantly lower than those in control group(P<0.05).Conclusion Salvianolate has significant clinical effect in the treatment of chronic heart failure,which can effectively improve the cardiac function of patients,and the expression of miR-21,miR-199a,Fas and FasL is better,and the safety is higher.
