摘 要:目的探讨不同剂量丙种球蛋白治疗小儿重症肺炎的疗效及对患儿免疫球蛋白、白细胞介素-6(IL-6)、C反应蛋白(CRP)的影响。方法选取2021年1月至2023年12月于泰安市妇幼保健院就诊的63例重症肺炎患儿为研究对象,按照使用丙种球蛋白剂量不同分为低剂量组(n=31)与高剂量组(n=32)。低剂量组在常规治疗基础上给予400 mg/(kg·d)丙种球蛋白,高剂量组在常规治疗基础上给予1000 mg/(kg·d)丙种球蛋白。比较两组患儿治疗有效率、免疫球蛋白[免疫球蛋白G(IgG)、免疫球蛋白A(IgA)、免疫球蛋白M(IgM)]水平、炎症因子、T淋巴细胞(CD4^(+)、CD4^(+)/CD8^(+))。结果高剂量组治疗有效率为93.75%,高于低剂量组的74.19%(P<0.05);治疗后两组IgG水平升高,且高剂量组高于低剂量组,差异有统计学意义(P<0.05);治疗后两组IL-6、CRP水平降低,且高剂量组低于低剂量组,差异有统计学意义(P<0.05);治疗后,两组CD4^(+)、CD4^(+)/CD8^(+)水平升高,且高剂量组高于低剂量组,差异有统计学意义(P<0.05)。结论采用1000 mg(kg·d)丙种球蛋白治疗小儿重症肺炎的疗效优于400 mg/(kg·d),可改善患儿免疫能力与机体炎症反应。Objective To explore the therapeutic effect of different doses of gamma globulin on children with severe pneumonia and the effects on immunoglobulin,interleukin-6(IL-6)and C-reactive protein(CRP).Methods Sixty-three children with severe pneumonia treated in Tai'an Maternal and Child Health Hospital from January 2021 to December 2023 were selected as the study objects,and divided into low-dose group(n=31)and high-dose group(n=32)according to different doses of gamma globulin.The low-dose group was given 400 mg/(kg·d)gamma globulin on the basis of conventional treatment,and the high-dose group was given 1000 mg/(kg·d)gamma globulin on the basis of conventional treatment.The treatment response rate,immunoglobulin[IgG],immunoglobulin A(IgA),immunoglobulin M(IgM)levels,inflammatory factors and T lymphocytes(CD4^(+),CD4^(+)/CD8^(+))were compared between the two groups.Results The effective rate of high dose group was 93.75%,which was higher than that of low dose group(74.19%)(P<0.05).After treatment,IgG levels of the two groups were increased,and the high dose group was higher than the low dose group,the difference was statistically significant(P<0.05).After treatment,the levels of IL-6 and CRP in the two groups were decreased,and the high dose group was lower than the low dose group,the difference was statistically significant(P<0.05).After treatment,the levels of CD4^(+)and CD4^(+)/CD8^(+)in 2 groups were increased,and the high dose group was higher than the low dose group,the difference was statistically significant(P<0.05).Conclusion The therapeutic effect of 1000 mg/(kg·d)gamma globulin in the treatment of severe pneumonia in children is better than 400 mg/(kg·d),which can improve the immune ability and inflammatory response of children.
