基于时机理论的早产儿父亲情感体验的质性研究

摘　　要：目的以时机理论为框架,对早产儿父亲在早产儿住院期间及出院居家早期不同阶段的情感体验及主要照护需求进行纵向研究,为制定针对性的个体化护理干预方案提供依据。方法采用目的抽样方法,根据纳入和排除标准,选取2023年8月~10月在青岛市某三级甲等医院新生儿科住院的54名处于不同阶段的早产儿父亲为研究对象,进行半结构式访谈,采用Colaizzi分析法分析资料并提炼主题。结果以时机理论为基础,提炼出早产儿父亲5个不同阶段情感体验的主题:(1)诊断期:情绪反应复杂,应对能力不足;(2)稳定期:缺乏疾病相关知识,心理负担重;(3)准备期:期待与担忧的矛盾情感体验;(4)调整期:独立照顾能力不足,渴望获得专业支持;(5)适应期:收获成长,希望得到专业支持。结论早产儿父亲在早产儿住院至出院居家早期不同阶段的体验及需求是动态变化的,医护人员应及时关注并给予早产儿父亲针对性个性化的干预措施,促进其心理健康和早产儿家庭的顺利过渡。Objective To conduct a longitudinal study on the emotional experience and main care needs of fathers of premature infants at different stages during hospitalization and early discharge at home based on"timing it right"theory,so as to provide a basis for formulating targeted individualized nursing intervention programs.Methods By the purposive sampling method,54 fathers of premature infants at different stages in NICU of a tertiary obstetrics and gynecology hospital in Qingdao from August 2023 to October 2023 were selected for semi-structured interviews,and the interview data were analyzed by Colaizzi phenomenon analyzing methods.Results Based on"timing it right"theory,the themes of the experiences and needs of premature fathers at 5 different stages were extracted.(1)Diagnosis period:complex emotional responses and insufficient coping skills.(2)Stable period:lack of disease-related knowledge and heavy psychological burden.(3)Discharge preparation period:conflicting emotional experiences of anticipation and worry.(4)Adjustment period:inadequate ability to care for independent and desire for professional support.(5)Adaptation period:harvest and grow,and hope to receive professional support.Conclusion The experiences and needs of fathers of premature infants at different stages of hospitalization and early home stay are dynamic,medical staff should promptly pay attention to and provide targeted and personalized intervention measures to fathers of premature infants to promote their mental health and smooth transition of their families.
