摘　　要：目的探究扬州市50岁及以上男性首次HIV抗体检测阳性时,其首阳检测原因及影响因素,为寻找改善50岁及以上男性艾滋病状况提供科学的参考和建议。方法采用问卷调查方式回顾性调查扬州市2016—2018年50岁及以上首次报告HIV抗体检测阳性的男性,运用Excel 2017和SPSS 26.0软件对数据进行整理分析,采用logistic回归分析和多重对应分析方法进行数据分析。结果161名调查对象首次HIV抗体检测阳性时,检测原因中主动要求仅占26.71%,医生建议占23.60%,常规检查占49.69%。logistic回归分析显示,月收入(OR=0.082、0.075,95%CI:0.008~0.817、0.007~0.796)、职业(OR=7.751,95%CI:1.676~35.838)、艾滋病和性病知识知晓状况(OR=6.822、6.843、0.212,95%CI:1.268~36.714、1.246~37.602、0.047~0.945)、就诊原因(OR=265.047、2462.569,95%CI:10.901~6444.111、63.584~95373.538)是50岁及以上男性HIV检测原因的影响因素;多重对应分析显示,主动要求检测与自我报告高危行为和企事业单位类工作人群有关联,医生建议检测与月收入≤1000元、艾滋病和性病知识不知晓、农民人群有关联,常规检查与月收入在1001~3000元和疾病就诊人群有关联。结论扬州市50岁及以上男性群体HIV主动检测率低,应对农村、中低收入、性病和艾滋病知晓率较低群体进行艾滋病和性病的宣传、免费检测、推广医务人员主动提供的艾滋病检测咨询服务。Objective To explore the reasons and influencing factors of the first positive HIV antibody test in Yangzhou City when men aged 50 years and above are positive for the first time,and to provide scientific references and suggestions for improving the status of AIDS in men aged 50 years and above.Methods A questionnaire survey was conducted to retrospectively investigate men aged 50 years and above who were reported to test positive for HIV antibodies for the first time in Yangzhou City from 2016 to 2018.Data were processed and analyzed using Excel 2017 and SPSS 26.0 software,and the logistic regression analysis and multiple correspondence analysis method were used to analyze the data.Results Of the 161 respondents who tested positive for HIV antibodies for the first time,only 26.71%of the reasons for testing were active requests,23.60%were doctor′s recommendations,and 49.69%were routine examinations.Logistic regression analysis showed that monthly income(OR=0.082,0.075,95%CI:0.008-0.817,0.007-0.796),occupation(OR=7.751,95%CI:1.676-35.838),awareness of AIDS and STD knowledge(OR=6.822,6.843,0.212,95%CI:1.268-36.714,1.246-37.602,0.047-0.945)and reasons for outpatient visit(OR=265.047,2462.569,95%CI:10.901-6444.111,63.584-95373.538)were the influencing factors of the reasons for HIV testing among men aged 50 years and above.Multiple correspondence analysis demonstrated that self-reported high-risk behaviors and individuals working in enterprises and institutions were related to active requested testing,monthly income of≤1000 yuan,and unawareness of AIDS and STD,and farmers were related to doctor-recommended testing,monthly income of 1001-3000 yuan and disease consultation populations were related to rutine examinations.Conclusion The actively received HIV testing rate among men aged 50 and above group in Yangzhou City is low.It is necessary to carry out AIDS and STD publicity,free testing,and HIV testing consultation services proactively provided by medical staff for populations who live in villages,have low to mi
