摘　　要：目的调查2023年某成年女性发生EB病毒(Epstein-Barr virus,EBV)感染与过敏性紫癜(henoch-sch9nlein purpura,HSP)是否由接种狂犬病疫苗所导致,是否存在因果关联。方法山东省菏泽市某县的县、乡镇两级专业人员对该病例进行个案调查,收集病例的相关资料;查询疫苗的质量批签发信息、接种方疫苗进货渠道、供货单位的资质证明等。组建该病例预防接种调查诊断专家组,对该疑似预防接种异常反应(adverse events following immunization,AEFI)病例开展调查诊断。结果该受种者于2023年6月27日10时,在某预防接种门诊接种人用狂犬病疫苗(Vero细胞)第3剂次,并按要求在留观室观察30min,现场无异常信息报告。该病例于2023年6月27日—2023年8月21日先后在4家医院就诊,主要诊断为HSP;该预防接种门诊运行及工作人员资质合法合规;预防接种各环节均符合相关法律法规及技术规范要求。该批号疫苗接种剂次5600剂次,异常反应发生率为0/10万剂。该病例不属于预防接种异常反应。结论该受种者接种狂犬病疫苗与发生EBV感染和HSP无因果关联,属于偶合症。Objective To investigate whether the occurrence of Epstein-Barr virus(EBV)infection and henoch-sch9nlein purpura(HSP)in an adult female was caused by rabies vaccination,and whether a causal association existed in 2023.Methods The case was investigated and relevant information on the case was collected by professionals at the county and township levels in a county of Heze City,Shandong Province.The quality batch issuance information of the vaccine,the vaccine purchase channel of the vaccinating party,and the qualification certificate of the supplying unit etcetera were inquired.A team of experts for investigation and diagnosis of the case was formed to carry out investigation and diagnosis of the case of adverse events following immunization(AEFI).Results The vaccine recipient received the third dose of human rabies vaccine(Vero cells)in a vaccination clinic at 10:00 on June 27,2023,and was observed in the observation room for 30 minutes as required,with no abnormal information reported at the site.The case was treated at four hospitals from June 27,2023 to August 21,2023,with a mainly diagnosed as HSP.The operation of the vaccination clinic and the qualifications of the staff were legal and compliant.All aspects of preventive vaccination complied with the requirements of relevant laws,regulations and technical specifications.The number of doses of the batch vaccine was 5600,and the incidence rate of abnormal reactions was 0/100000 doses.This case did not belong to an abnormal reaction to vaccination.Conclusion There is no causal correlation between the vaccination of rabies vaccine and the occurrence of EBV infection and HSP in this recipient,which is a coupling disease.
