基层医疗机构软式内镜再处理现状调查研究

摘　　要：目的深入了解基层医疗机构软式内镜再处理现况,分析其存在的问题,提出针对性整改措施以提升软式内镜清洗消毒质量。方法2023年7月采用定性研究访谈和现场查检相结合的研究方法,借助管腔器械可视系统对湖北省襄阳市某地区8家基层医疗机构软式内镜内部清洗消毒质量情况进行查检,率的比较采用χ^(2)检验。结果现场访谈软式内镜清洗消毒人员32人,年龄40~<50岁的26人,占81.25%;中级及以上职称的14人,占43.75%。胃镜、肠镜、支气管镜清洗消毒人员每年平均培训次数分别为3次、3次、1次。现场查检胃肠镜、喉镜、支气管镜存在问题率分别为43.75%、75.00%、62.50%,差异无统计学意义(χ^(2)=3.23,P=0.199)。存在的问题主要为胃肠镜内部有水珠,内壁有锈渍和污渍;喉镜管腔内壁有较多水渍,内壁有红色污渍;支气管镜管腔内壁划痕较多,内壁有水渍、红色污渍,内镜出口边缘粗糙有磨损等。结论基层医疗机构软式内镜清洗消毒质量和清洗消毒人员专业能力有待进一步提高,喉镜清洗消毒设施设备有待进一步完善,软式内镜清洗及干燥质量值得密切关注。Objective To gain an in-depth understanding of the current situation of reprocessing of flexible endoscopes in primary healthcare institutions,analyze the existing problems,and propose targeted rectification measures to improve the quality of cleaning and disinfection of flexible endoscopes.Methods In July 2023,a combination of qualitative research interviews and on-site inspections was used to consult the quality of internal cleaning and disinfection of flexible endoscopes in 8 primary healthcare institutions in a certain area of Xiangyang City,Hubei Province,using a lumen visualization system.The comparison of rates was conducted using theχ^(2)test.Results A total of 32 personnel responsible for cleaning and disinfecting flexible endoscopes were interviewed on-site,with 26 individuals in the age of 40-<50 years old,accounting for 81.25%,and 14 individuals holding intermediate or higher professional titles,accounting for 43.75%.The average annual training frequencies for personnel responsible for cleaning and disinfecting gastroscopes,colonoscopes,and bronchoscopes were 3 times,3 times,and 1 time,respectively.The problem rates identified during on-site inspections for gastroscopes,laryngoscopes,and bronchoscopes were 43.75%,75.00%,and 62.50%,respectively,with no statistically significant differences(χ^(2)=3.23,P=0.199).The main issues identified included the presence of water droplets inside the gastrointestinal scopes,rust and stain marks on the inner walls;more water stains on the inner walls and red stains on the inner wall of laryngoscopes;numerous scratches on the inner walls of bronchoscopes,water stains,red stains,and rough edges with wear on the exit edge of the endoscope etcetera.Conclusion The quality of cleaning and disinfection of flexible endoscopes in primary healthcare institutions and the professional capabilities of cleaning and disinfection personnel need further improvement.The cleaning and disinfection facilities and equipment for laryngoscopes need to be further improved,and attention shou
