自护依从性在胆道带T管患者出院准备度与社会支持的中介效应研究

摘　　要：目的:了解胆道手术后带T管出院患者出院准备度的现状,探讨自护依从性在社会支持与出院准备度之间的中介效应。方法:采用便利抽样法抽取芜湖市某三级甲等医院肝胆外科235例带T管出院患者,使用出院准备度量表、社会支持量表、T管患者自我护理行为依从性量表进行调查,采用Spearman相关性分析出院准备度、社会支持、自护依从性间的关系,运用PROCESS程序检验自护依从性在社会支持与出院准备度之间是否存在中介作用。结果:胆道带T管患者出院准备度84(75,93)分,社会支持43(40,48)分,自护依从性4(3,6)分,社会支持、自护依从性与出院准备度均存在正相关(P<0.05);自护依从性在社会支持和出院准备度之间存在部分中介作用,中介效应占总效应的20.55%。结论:胆道术后带T管出院患者自护依从性在社会支持和出院准备度之间存在部分中介作用,护理人员应采取有效措施提高患者的社会支持水平和自护依从性,从而提高患者的出院准备度。Objective:To understand the current situation of discharge readiness of patients discharged with T-tubes after biliary surgery and to explore the mediating effect of self-care compliance between social support and discharge readiness.Methods:Using the convenience sampling method to select 235 patients discharged with T-tubes from the Department of Hepatobiliary Surgery of a tertiary hospital and a survey was conducted using the discharge readiness scale,social support scale,and T-tube patients with self-care behavior compliance scale,and the Spearman correlation analysis was used to analyze the relationship between discharge readiness,social support,self-care compliance,using the PROCESS program to test whether there is an intermediary effect between social support and discharge readiness.Results:The scores of Discharge readiness was 84(75,93),43(40,48)for social support,self-care adherence was 4(3,6),and a positive correlation between social support,self-care adherence,and discharge readiness(P<0.05).Self-care adherence was partially mediated between social support and readiness for discharge,with the mediation effect accounting for 20.55% of the total effect.Conclusion:Self-care compliance between social support and discharge readiness,and nursing staff should take effective measures to improve the level of social support and self-care compliance for discharge.
