摘 要:目的研究微量营养素配合饮食指导对桥本甲状腺炎患者甲状腺激素水平、营养指标的影响。方法选取2021年3月至2023年3月在南京医科大学第二附属医院就诊的103例桥本甲状腺炎患者作为研究对象,按照随机数字表法分为研究组(51例)和常规组(52例)。常规组施以饮食指导,研究组施以微量营养素配合饮食指导。比较甲状腺激素水平、甲状腺自身免疫性抗体指标[促甲状腺激素(TSH)、游离三碘甲状腺原氨酸(FT_(3))及游离甲状腺素(FT_(4))]、营养指标[甲状腺过氧化物酶抗体(TPOAb)、甲状腺球蛋白抗体(TGAb)]、治疗效果和生活质量评分。结果干预3个月和干预6个月后,两组FT_(3)、FT_(4)水平均降低,且研究组低于常规组,TSH水平均上升,且研究组高于常规组(P<0.05)。干预6个月后,研究组TPOAb、TGAb水平均低于常规组(P<0.05)。干预3个月和干预6个月后,研究组血清白蛋白、血红蛋白、转铁蛋白均高于常规组(P<0.05)。研究组治疗总有效率高于常规组(P<0.05)。两组生活质量评分中心理功能、躯体功能、认知功能、社会功能评分均高于常规组(P<0.05)。结论对桥本甲状腺炎患者而言,采用微量营养素配合饮食指导,这有助于改善患者的甲状腺功能,提高营养水平及生活质量,应用效果显著。Objective To study the effects of micronutrients combined with dietary guidance on thyroid hormone levels and nutritional indexes in Hashimoto thyroiditis patients.Methods A total of 103 patients with Hashimoto’s thyroiditis treated at the Second Affiliated Hospital of Nanjing Medical University from March 2021 to March 2023 were selected as the study subjects.They were divided into the study group(51 cases)and the conventional group(52 cases)using a random number table.The conventional group received dietary guidance,while the study group received micronutrient supplementation combined with dietary guidance.The thyroid hormone levels,thyroid autoantibody indicators[thyroid-stimulating hormone(TSH),free triiodothyronine(FT_(3)),and free thyroxine(FT_(4))],nutritional indicators[thyroid peroxidase antibody(TPOAb)and thyroglobulin antibody(TGAb)],treatment efficacy,and quality of life scores were compared between the two groups.Results After 3 months and 6 months of intervention,the levels of FT_(3) and FT_(4) decreased in both groups,with the study group showing lower levels than the conventional group,while the TSH levels increased in both groups,with the study group showing higher levels than the conventional group(P<0.05).After 6 months of intervention,the levels of TPOAb and TGAb in the study group were lower than those in the conventional group(P<0.05).After 3 months and 6 months of intervention,the serum albumin,hemoglobin,and transferrin levels in the study group were higher than those in the conventional group(P<0.05).The total effective rate of treatment in the study group was higher than that in the conventional group(P<0.05).The quality of life scores,including psychological function,physical function,cognitive function,and social function,were higher in the study group than in the conventional group(P<0.05).Conclusions For patients with Hashimoto’s thyroiditis,the use of micronutrients combined with dietary guidance can help improve thyroid function,enhance nutritional levels,and improve quality of
