摘　　要：目的 比较腹腔镜腹膜前疝修补术(TAPP)与传统腹膜前疝修复术在老年腹股沟疝患者中的疗效。方法 选取2022年1月至2024年1月哈尔滨二四二医院60例老年腹股沟疝患者随机分为试验组和对照组,每组30例。试验组采用TAPP手术,对照组接受开放式修补。对比两组手术时长、失血量、住院时间、疼痛程度(VAS评分)及并发症发生情况、复发率及生活质量评分。结果 试验组手术时间及住院时间较短于对照组,术中出血量少于对照组(P <0.05)。术后1 d和术后1周,试验组VAS评分均低于对照组(P <0.05)。试验组并发症总发生率及复发率均低于对照组(P <0.05)。术后第1个月、第3个月及第6个月试验组患者的生活质量总分均高于对照组(P <0.05)。结论 TAPP手术在老年腹股沟疝治疗中展现出一定优势,可减少手术创伤、缩短康复周期、减轻早期疼痛感、降低并发症的发生风险,提高短期内的生活质量。Objective To compare the efficacy of laparoscopic transabdominal preperitoneal hernia repair(T)and traditional open preperitoneal hernia repair in elderly patients with inguinal hernia.Methods A total of 60 elderly patients with inguinal hernia in Harbin 242 Hospital from January 2022 to January 2024 were randomly divided into the trial group and the control group,with 30 cases in each group.The trial group was treated with TAPP surgery,and the control group received open repair.The operation time,blood,hospital stay,pain degree(VAS score),and the incidence of complications,recurrence rate,and quality of life score were compared between the two groups.Results The operation time and hospital stay in the trial group were significantly shorter than those in the control group,and the intraoperative blood loss was less than that in the control group(P<0.05).At 1 day and 1 week postoperatively,the VAS scores in the trial group were significantly lower than those in the control group(P<0.05).The overall incidence of complications and recurrence rate in the trial group were significantly lower than those in the control group(P<0.05).At 1 month,3 months,and 6 months postoperatively,the total quality of life scores in the trial group were significantly higher than those in the control group(P<0.05).Conclusions TAPP surgery shows obvious advantages in the of elderly inguinal hernia,which can reduce surgical trauma,shorten the recovery cycle,reduce early pain,lower the risk of complications,and improve the quality life in the short term.
