某院2021-2023年地佐辛使用情况合理性分析

摘　　要：目的调查惠州市中大惠亚医院2021-2023年地佐辛注射液的使用情况,分析其用药合理性。方法通过医院信息管理系统系统抽取2021年1月至2023年12月惠州市中大惠亚医院住院部各科室地佐辛注射液的使用情况,包括患者信息、用药量及诊断详情,按年度整理临床科室的地佐辛注射液应用数据。采用限定日剂量(DDD)方法,对3年内地佐辛注射液的使用情况进行比较分析。因该医院领取地佐辛需要红处方,随机抽取每年300张处方进行点评,总结患者用药特点及评估临床科室使用地佐辛的合理性。结果使用地佐辛注射液的临床科室主要为手术室、骨科一区、骨科二区、内镜中心、外科一区。其中手术室的用药频度(DDDs)值分别为2021年1233.38,2022年1152.75,2023年986.63,高于其他科室;其药物利用指数(DUI)值分别为2021年3.38,2022年3.16,2023年3.21,其余各科室的药物利用指数(DUI)值均<1。分析地佐辛注射液的临床应用案例,结果显示其主要在围手术期镇痛方面发挥重要作用。统计数据显示,2021年300例接受地佐辛注射液治疗的患者中,不合理用药案例为8例,占比2.67%;至2022年,该数字在同样300例患者中增至10例,占比3.33%;而到了2023年,尽管总数仍为300例,不合理用药情况略有下降,为7例,占比2.3%。结论某医院地佐辛注射液的使用频率偏高。手术室的DUI值均>1,提示可能存在不合理应用情况。临床药师需与临床医师紧密协作,并适时开展处方专项评估,以保障临床用药的有效性、合理性和安全性。Objective Investigate the use of Dizocin injection in Huizhou Zhongda Huiya Hospital from 2021 to 2023 and analyze its rational use.Methods A total of 104 cases of dizocine injection use in inpatient departments of Huizhou Sun Yat-sen Huayu Hospital from January 2021 to December 2023 were extracted through the hospital information management system.The data included patient information,dosage,and diagnostic details,and were organized by clinical departments on an annual basis.The defined daily dose(DDD)method was used to compare and analyze the use of dizocine injection over the three years.Since the hospital required a red prescription for the dispensing of dizocine,300 prescriptions were randomly selected each year for review to summarize the characteristics of patient medication use and to evaluate the rationality of dizocine use in clinical departments.Results The clinical departments using Dizocine Injection were mainly the Operating Room,Orthopedic Ward I,Orthopedic Ward II,Endoscopy Center,and Surgical Ward I,among others.The drug utilization frequency(DDDs)values for the Operating Room were 1233.38 in 2021,1152.75 in 2022,and 986.63 in 2023,which were higher than those of other departments.The drug utilization index(DUI)values for the Operating Room were 3.38 in 2021,3.16 in 2022,and 3.21 in 2023,while the DUI values for all other departments were less than 1.Analysis of the clinical application cases of Dizocine Injection showed that it primarily played an important role in perioperative analgesia.Statistical data indicated that among the 300 patients treated with Dizocine Injection in 2021,there were 8 cases of irrational drug use,accounting for 2.67%.In 2022,this number increased to 10 cases among the same number of patients(300),representing 3.33%.By 2023,although the total number remained at 300,the number of irrational drug use cases slightly decreased to 7,accounting for 2.3%.Conclusions The frequency of use of Dizocin injection in a certain hospital is relatively high.The DUI values in the operati
