骨折联络服务的智慧护理模式对髋部脆性骨折患者继发骨折风险的影响

摘　　要：目的 分析髋部脆性骨折患者开展骨折联络服务的智慧护理模式对继发骨折风险的影响。方法 选取厦门市第三医院2022年8月至2023年8月确诊的髋骨脆性骨折患者94例,按照随机数字表法随机划分为两组,每组47例。对照组实施常规延续护理,观察组实施骨折联络服务的智慧护理模式。从用药依从性、知识掌握度、髋关节功能、日常生活能力、再骨折情况对两组的护理效果进行综合评价。结果 干预6个月时,观察组Morisky用药依从性问卷(MMAS-8)得分较对照组更高(P <0.05)。干预6个月时,两组中文版骨质疏松知识工具(OKT)得分高于干预前(P <0.05)。观察组较OKT得分对照组更高,统计学有意义(P <0.05)。干预6个月时,两组Harris髋关节评分、Barthel指数(BI)得分高于干预前(P<0.05)。观察组Harris髋关节评分、BI得分较对照组更高(P<0.05)。干预3个月、干预6个月时,观察组再骨折发生率较对照组低(P <0.05)。结论 髋部脆性骨折患者开展骨折联络服务的智慧护理模式有助于提高用药依从性,加深知识掌握度,改善髋关节功能及日常生活能力,降低再骨折风险。Objective To analyze the effect of intelligent nursing mode of fracture liaison service on the risk of secondary fracture in patients with hip fragility fracture.Methods A total of 94 patients with hip fragility fractures diagnosed at the Third Hospital of Xiamen from August 2022 to August 2023 were included in the study.These patients were randomly divided into two groups using a random number table,with 47 cases in each group.The control group received routine extended care,while the observation group was provided with a smart nursing model based on fracture liaison services.The nursing outcomes of the two groups were comprehensively evaluated based on medication adherence,knowledge acquisition,hip joint function,activities of daily living,and incidence of re-fracture.Results At the 6-month follow-up,the scores on the Morisky Medication Adherence Scale(MMAS-8)in the observation group were significantly higher than those in the control group(P<0.05).At the 6-month follow-up,the scores on the Chinese version of the Osteoporosis Knowledge Test(OKT)in both groups were significantly higher than before the intervention(P<0.05).The OKT scores in the observation group were significantly higher than those in the control group,with statistical significance(P<0.05).At the 6-month follow-up,the Harris Hip Scores and Barthel Index(BI)scores in both groups were significantly higher than before the intervention(P<0.05).The Harris Hip Scores and BI scores in the observation group were significantly higher than those in the control group(P<0.05).At the 3-month and 6-month follow-ups,the incidence of re-fracture in the observation group was significantly lower than that in the control group(P<0.05).Conclusions The smart nursing model of fracture liaison service for patients with hip fragility fractures helps to improve medication compliance,deepen knowledge mastery,improve hip function and daily living ability,and reduce the risk of refracture.
