老年全膝关节置换术术后镇痛分别应用盐酸曲马多和氟比洛芬酯注射液的效果观察

摘　　要：目的:探究氟比洛芬酯注射液在老年全膝关节置换术后镇痛效果及对患者应激反应的影响。方法:选取2022年9月—2024年9月我院接受治疗的72例老年全膝关节置换术患者,运用抽签法平均分为两组,每组36例。参照组应用盐酸曲马多治疗,研究组应用氟比洛芬酯注射液治疗。比较两组疼痛程度(VAS评分)、炎症应激反应[C反应蛋白(CRP)]、不良反应、膝关节主动活动度、膝关节功能。结果:研究组术后6h、术后12h、术后24h、术后48h的VAS评分均低于参照组(P<0.05)。研究组拔管即刻、术后6h、术后12h、术后24h、术后48h的血清CRP水平与参照组比较,差异无统计学意义(P>0.05),而两组术后6h、术后12h、术后24h、术后48h的血清CRP水平均低于拔管即刻(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。研究组术后48h膝关节主动活动度高于参照组(P<0.05)。研究组出院时膝关节功能评分高于参照组(P<0.05)。结论:氟比洛芬酯注射液治疗能减轻老年全膝关节置换术患者术后疼痛,改善膝关节功能,且不会引起手术应激反应,不良反应少,治疗效果显著,值得临床应用。Objective:To investigate the analgesic effect of flurbiprofen axetil injection after total knee arthroplasty in elderly patients and its effect on stress response.Methods:A total of 72 elderly patients undergoing total knee arthroplasty in our hospital from September 2022 to September 2024 were selected and divided into two groups with 36 patients in each group by lottery method.The control group was treated with tramadol hydrochloride,and the study group was treated with flurbiprofen axetil injection.The pain level(VAS score),inflammatory stress response[C-reactive protein(CRP)],adverse reactions,knee active range of motion,and knee function were compared between the two groups.Results:The VAS score of the study group at 6 h,12 h,24 h and 48 h after operation was lower than that of the reference group(P<0.05).There was no significant difference between the serum CRP levels in the study group and the control group at the time of extubation,6 h,12 h,24 h and 48 h after extubation(P>0.05),while the serum CRP levels in the two groups were lower than those at the time of extubation at 6 h,12 h,24 h and 48 h after extubation(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).The active range of motion of the knee joint in the study group at 48 hours after surgery was higher than that in the control group(P<0.05).The knee function score of the study group at discharge was higher than that of the reference group(P<0.05).Conclusion:Flurbiprofen axetil injection can reduce postoperative pain and improve knee joint function in elderly patients undergoing total knee arthroplasty,and will not cause surgical stress reaction,with few adverse reactions,and the treatment effect is remarkable,which is worthy of clinical application.
