摘 要:目的:研究经外周静脉穿刺中心静脉置管(PICC)化疗的恶性肿瘤患者实施握球运动+水胶体敷料干预后的效果。方法:选取2021年3月—2024年3月我院收治的104例接受PICC置管并化疗的恶性肿瘤患者。采用随机数字表法将患者分为对照组和观察组,每组52例。对照组接受握球运动干预加普通敷贴,而观察组则在握球运动干预基础上加用水胶体敷料。比较两组腋静脉血液最大流速(V_(max))、平均血液流速(TMFV)、血管直径和血管压力,并记录并发症发生率。结果:观察组干预7 d后V_(max)、TMFV均高于对照组,且干预后1个月V_(max)、TMFV、血管大小及压力均高于对照组(P<0.05);观察组并发症发生率低于对照组(P<0.05)。结论:握球运动联合水胶体敷料不仅能有效改善恶性肿瘤PICC置管侧肢体的血液循环,还能降低机械性静脉炎和导管相关性血栓(CRT)发生风险。Objective:To study the effect of ball-holding exercise+hydrocolloid dressing intervention on malignant tumor patients undergoing peripherally inserted central catheter(PICC)chemotherapy.Methods:A total of 104 patients with malignant tumors who received PICC catheterization and chemotherapy in our hospital from March 2021 to March 2024 were selected.The patients were divided into control group and observation group by random number table method,with 52 patients in each group.The control group received ball-holding exercise intervention plus ordinary application,while the observation group added hydrocolloid dressing on the basis of ball-holding exercise intervention.The maximum blood flow velocity(V_(max)),mean blood flow velocity(TMFV),vascular diameter and vascular pressure of axillary vein were compared between the two groups,and the incidence of complications was recorded.Results:The V_(max) and TMFV of the observation group were higher than those of the control group after 7 days of intervention,and the V_(max),TMFV,vascular size and pressure were higher than those of the control group after 1 month of intervention(P<0.05).The incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion:Ball-holding exercise combined with hydrocolloid dressing can not only effectively improve the blood circulation of PICC catheter side limbs in malignant tumors,but also reduce the risk of mechanical phlebitis and catheter-related thrombosis(CRT).
