摘　　要：目的:探讨以家庭为中心的个案干预应用在2型糖尿病伴心功能不全患者药物治疗期间的效果。方法:选取2022年6月—2024年10月我院102例T2DM患者为研究对象,遵循随机化原则分为观察组(n=51)、对照组(n=51)。对照组给予常规干预,观察组在对照组的基础上增加以家庭为中心的个案干预。观察两组患者血糖及体重控制情况、胰岛功能、心功能及干预期间的药物不良反应。结果:干预3个月后,两组FPG、2hPG、HbA1c、BMI均降低,且观察组低于对照组(P<0.05);干预3个月后,两组FINS、HOMAIR均降低,HOMA-β均升高,且观察组优于对照组(P<0.05);干预3个月、6个月,两组LVEF、SV、CI水平均较干预前升高,且观察组高于对照组(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:以家庭为中心的个案干预可以有效控制2型糖尿病伴心功能不全患者的血糖水平及胰岛功能,同时保护其心功能,减轻不良反应。Objective:To explore the effect of family-centered case nursing combined with dapagliflozin on type 2 diabetes mellitus with cardiac insufficiency and its effect on cardiac function.Methods:A total of 102 patients with T2 DM in our hospital from June 2022 to October 2024 were selected as the research objects.According to the principle of randomization,they were divided into observation group(n=51)and control group(n=51).The control group was given routine lipid-lowering intervention.The observation group added family-centered case nursing on the basis of the control group.The blood glucose and weight control,islet function,cardiac function and adverse drug reactions during intervention were observed in the two groups.Results:After 3 months of intervention,FPG,2 hPG,HbA1 c and BMI in the two groups decreased,and those in the observation group were lower than the control group(P<0.05).After 3 months of intervention,FINS and HOMA-IR in the two groups decreased,HOMA-βincreased,and those in the observation group were better than the control group(P<0.05).After 3 months and 6 months of intervention,the levels of LVEF,SV and CI in the two groups were higher than those before intervention,and those in the observation group were higher than the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Family-centered case nursing combined with dapagliflozin intervention can effectively control the blood glucose level and islet function of patients with type 2 diabetes mellitus and cardiac insufficiency,protect their cardiac function and reduce adverse reactions.
