摘  要:目的:探讨新生儿呼吸窘迫综合征(NRDS)治疗中采用雾化肺表面活性物质联合枸橼酸咖啡因治疗的价值。方法:于2022年6月-2024年6月期间选择我院收治的NRDS患儿84例,以随机数字表法分2组,各42例。对照组采用雾化肺表面活性物质治疗,观察组采用雾化肺表面活性物质联合枸橼酸咖啡因治疗,比较两组呼吸力学指标[气道阻力(R)、内源性呼气末正压(iPEEP)、静态肺顺应性(Cst)]、血气指标[氧分压(PaO_(2))、二氧化碳分压(PaCO_(2))、p H值]、氧化应激指标[超氧化物歧化酶(SOD)、丙二醛(MDA)]、[血清铁蛋白(SF)、C反应蛋白(CRP)]。结果:治疗后,观察组R、iPEEP、PaCO_(2)、MDA、SF、CRP低于对照组,Cst、PaO_(2)、SOD高于对照组(P<0.05)。结论:NRDS治疗中应用雾化肺表面活性物质联合枸橼酸咖啡因可促进患儿呼吸力学指标、血气分析指标、氧化应激指标改善,也能改善免疫及炎症指标水平。Objective:To explore the value of nebulized pulmonary surfactant combined with caffeine citrate in the treatment of neonatal respiratory distress syndrome(NRDS).Methods:A total of 84 children with NRDS admitted to our hospital from June 2022 to June 2024 were selected and divided into two groups by random number table method,with 42 children in each group.The control group was treated with aerosolized pulmonary surfactant,and the observation group was treated with aerosolized pulmonary surfactant combined with caffeine citrate.The respiratory mechanics indexes[airway resistance(R),endogenous positive end-expiratory pressure(iPEEP),static lung compliance(Cst)],blood gas indexes[oxygen partial pressure(PaO_(2)),carbon dioxide partial pressure(PaCO_(2)),pH value],oxidative stress indexes[superoxide dismutase(SOD),malondialdehyde(MDA)],[serum ferritin(SF),C-reactive protein(CRP)]were compared between the two groups.Results:After treatment,R,iPEEP,PaCO_(2),MDA,SF and CRP in the observation group were lower than those in the control group,and Cst,PaO_(2) and SOD were higher than those in the control group(P<0.05).Conclusion:The application of nebulized pulmonary surfactant combined with caffeine citrate in NRDS treatment can promote the improvement of respiratory mechanics indicators,blood gas analysis indicators,oxidative stress indicators,as well as immune and inflammatory indicators in children.
