足底砭石联合局部推拿干预在慢性非特异性下腰痛的应用效果

摘　　要：目的:观察足底砭石联合推拿对慢性非特异性下腰痛(CNLBP)患者焦虑情绪及血清神经肽Y(NPY)、血管活性肠肽(VIP)水平的影响。方法:纳入河南中医药大学第三附属医院2023年5月—2024年5月就诊的60例CNLBP患者,采用随机数字表法分组,对照组30例、试验组30例。对照组采用常规推拿法,试验组在对照组基础上加用足底砭石疗法,比较两组Oswestry功能障碍指数(ODI)、汉密尔顿焦虑量表(HAMA)、焦虑自评量表(SAS)评分及血清NPY、VIP水平。结果:试验组干预后ODI、HAMA、SAS评分低于对照组(P<0.05);试验组干预后血清脑肠肽NPY水平高于对照组,VIP水平低于对照组(P<0.05)。结论:足底砭石联合推拿可改善慢性非特异性下腰痛患者血清NPY、VIP水平,有助于缓解患者焦虑情绪,改善临床症状。Objective:To observe the effect of plantar stone needle combined with massage on anxiety and serum neuropeptide Y(NPY)and vasoactive intestinal peptide(VIP)levels in patients with chronic nonspecific low back pain(CNLBP).Methods:A total of 60 patients with CNLBP who were treated in the Third Affiliated Hospital of Henan University of Traditional Chinese Medicine from May 2023 to May 2024 were included.They were divided into control group(30 patients)and experimental group(30 patients)by random number table method.The control group was treated with routine massage,and the experimental group was treated with plantar stone needle therapy on the basis of the control group.The Oswestry dysfunction index(ODI),Hamilton anxiety scale(HAMA),self-rating anxiety scale(SAS)scores and serum NPY and VIP levels were compared between the two groups.Results:The scores of ODI,HAMA and SAS in the experimental group were lower than those in the control group(P<0.05).After intervention,the level of serum brain gut peptide NPY in the experimental group was higher than that in the control group,and the level of VIP was lower than that in the control group(P<0.05).Conclusion:Plantar stone needle combined with massage can improve the levels of serum NPY and VIP in patients with chronic nonspecific low back pain,help to alleviate the anxiety of patients and improve clinical symptoms.
