热奄包联合止痛消炎软膏膏摩方案对膝关节置换术后下肢瘀血肿胀疼痛的效果观察

摘　　要：目的:探究热奄包联合止痛消炎软膏膏摩技术对膝关节置换术(TKA)后患者下肢瘀血、肿胀、疼痛情况的影响。方法:选取2022年11月—2023年5月我院100例接受TKA患者,采用随机抽签法分为对照组、试验组,各50例。对照组采用常规护理,试验组采用常规护理+热奄包联合止痛消炎膏膏摩技术干预,两组均干预7 d。对比两组不良反应、视觉模拟评分(VAS)、患侧膝关节髌骨上下围周径、双侧皮温差值、膝关节功能(HSS)评分及中医证候积分。结果:试验组术后3 d、7 d、14 d VAS评分均低于对照组,差异有统计学意义(P<0.05);试验组术后3 d、7 d、14 d膝关节髌骨上围、下围均小于对照组,差异有统计学意义(P<0.05);试验组术后3 d、7 d、14 d皮温差值均小于对照组,差异有统计学意义(P<0.05);试验组术后7 d HSS评分高于对照组,中医证候积分低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:热奄包联合止痛消炎软膏膏摩干预能缓解TKA后患者疼痛,减轻肿胀,促进膝关节功能恢复,安全有效。Objective:To explore the effect of hot package combined with Zhitong Xiaoyan ointment rubbing technique on lower limb blood stasis,swelling and pain in patients after knee arthroplasty(TKA).Methods:From November 2022 to May 2023,100 patients undergoing TKA in our hospital were selected and randomly divided into control group and experimental group,with 50 patients in each group.The control group was treated with routine nursing,and the experimental group was treated with routine nursing+hot package combined with Zhitong Xiaoyan ointment rubbing technique.Both groups were intervened for 7 days.The adverse reactions,visual analogue scale(VAS),upper and lower circumference of patella,bilateral skin temperature difference,knee function(HSS)score and TCM syndrome score were compared between the two groups.Results:The VAS scores of the experimental group at 3 d,7 d and 14 d after operation were lower than those of the control group,and the differences were statistically significant(P<0.05).The upper and lower circumferences of the knee joint in the experimental group were smaller than those in the control group at 3 d,7 d and 14 d after operation,and the differences were statistically significant(P<0.05).The skin temperature difference of the experimental group was lower than that of the control group at 3 d,7 d and 14 d after operation,and the difference was statistically significant(P<0.05).The HSS score of the experimental group was higher than that of the control group at 7 days after operation,and the TCM syndrome score was lower than that of the control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Hot package combined with Zhitong Xiaoyan ointment can relieve pain,reduce swelling and promote the recovery of knee joint function in patients after TKA,which is safe and effective.
