摘　　要：复杂性肛瘘是肛肠外科常见的疾病,临床上以手术治疗为主,由于病变位置高、瘘管弯曲和存在死腔等因素,术后容易出现复发和大便失禁等并发症。手术治疗复杂性肛瘘的原则为在治愈肛瘘的同时减少肛门功能损伤。在国外,视频辅助肛瘘治疗技术日益普及,这种技术通过内镜直视,准确识别内口和瘘管路径,并在不切开瘘管的情况下清除病灶组织,是一种微创手术方式,具有创伤小、痛苦轻、恢复快等优势。但其治疗费用昂贵,设备要求高,在国内处于起步阶段,因此,缺乏大量的相关研究,本文收集该技术治疗复杂性肛瘘的相关研究作一综述,以指导临床应用。Complex anal fistula is a common disease in colorectal surgery and is mainly treated surgically.Due to factors such as the high location of the lesion,the curvature of the fistula tract and the presence of dead space,recurrence and postoperative complications such as fecal incontinence are prone to occur.The principle of surgical treatment for complex anal fistulas has become to cure the fistula while minimize damage to anal function.Video-assisted anal fistula treatment technology is increasingly popular abroad.This technology,through endoscopic visualization,accurately identifies the internal opening and the path of the fistula tract and removes the diseased tissue without incising the fistula tract.It is a minimally invasive surgical method with the advantages of minimal trauma,less pain,and rapid recovery.However,the treatment is expensive and the equipment requirements are high.Video-assisted anal fistula treatment technology is still in its infancy in China,and thus,there is a lack of extensive related research.This article reviews the relevant studies on the treatment of complex anal fistulas with this technology to guide clinical application.
