依诺肝素预防阵发性室上性心动过速导管消融术后下肢深静脉血栓的疗效

摘　　要：目的 评估阵发性室上性心动过速(PSVT)射频导管消融术后应用依诺肝素预防下肢深静脉血栓(DVT)的疗效。方法 2022年1月至2024年1月期间,纳入300例接受导管消融的PSVT患者,分为术后未抗凝的对照组(162例)和术后4 h接受抗凝治疗的依诺肝素组(138例),比较两组术后DVT发生率的差异。结果 术后第1 d共7.7%的患者(23例)发生了DVT,其中肌间静脉血栓7例,股静脉穿刺点血栓16例。经统计分析,依诺肝素组的DVT发生率为3.6%,显著低于对照组的11.1%。尽管两组间肌间静脉血栓发生率无差异,但依诺肝素组股静脉穿刺点血栓的发生率为2.2%,低于对照组的8.0%。结论 术后4 h给予4 000 U依诺肝素皮下注射可有效降低DVT发生风险,但预防肌间静脉血栓效果不明显。Objective To evaluate the efficacy of enoxaparin in preventing lower limb deep vein thrombosis(DVT)following radiofrequency catheter ablation for paroxysmal supraventricular tachycardia(PSVT).Methods From January 2022 to January 2024,300 patients undergoing catheter ablation for PSVT were enrolled.They were divided into control group(162 patients)without anticoagulant therapy using enoxaparin and enoxaparin group(138 patients)by receiving enoxaparin anticoagulant therapy post-operation 4 hours,to compare the post-operative DVT incidence rates between the two groups.Results On the first day of post-operation,7.7%of patients(23 cases)were DVT by ultrasound detection,including 7 cases of muscular vein thrombosis and 16 cases of femoral vein puncture site thrombosis.The incidence rate of DVT in the enoxaparin group was 3.6%,significantly lower than l1.1%in the control group.Although there was no significant difference for the incidence rates of muscular vein thrombosis between two groups,the occurrence rate of femoral vein puncture site thrombosis in the enoxaparin group was 2.2%,lower than 8.0%in the control group.Conclusion Administering 4 ooo U of enoxaparin subcutaneously post-operation 4 hours effectively reduces the risk of DVT,although its preventive effect on muscular vein thrombosis is unconspicuous.
