利福喷丁在肺结核中的疗效及对Th1/Th2细胞因子的影响研究

摘　　要：目的分析利福喷丁在肺结核患者中的疗效及对Th1/Th2细胞因子的影响。方法选取2019年3月至2020年7月154例在本院治疗的肺结核患者为对象,平均分为观察组与对照组(均77例)。对照组进行常规化疗,观察组的化疗方案相同,差异在于利用利福喷丁替代利福平。对比二组的疗效以及治疗前后Th1细胞因子(白细胞介素2-IL-2、干扰素γ-INF-γ)和Th2细胞因子(白细胞介素4-IL-4、白细胞介素6-IL-6)的变化。结果观察组与对照组的总治疗有效率分别为90.9%和77.9%,差异有统计学意义(χ^(2)值为4.936,P值为0.026)。观察组与对照组治疗前IL-2分别为(20.2±4.2)pg/m L和(20.1±4.3)pg/m L,INF-γ分别为(14.9±2.3)pg/m L和(15.0±2.4)pg/m L,两组治疗前Th1细胞因子指标的比较差异无统计学意义(t值为0.146、0.264,P值为0.442、0.396),观察组与对照组治疗后IL-2分别为(14.6±2.2)pg/m L和(17.3±3.5)pg/m L,INF-γ分别为(11.0±1.1)pg/m L和(13.5±1.7)pg/m L,观察组治疗后Th1细胞因子明显低于对照组(t值为5.731、10.834,P值为0.000、0.000)。观察组与对照组治疗前IL-4分别为(19.8±4.2)pg/m L和(20.1±4.0)pg/m L,IL-6分别为(952.6±112.3)pg/m L和(961.5±115.4)pg/m L,两组治疗前Th2细胞因子指标的比较差异无统计学意义(t值为0.449、0.485,P值为0.327、0.314),观察组与对照组治疗后IL-4分别为(18.6±1.5)pg/m L和(19.2±2.2)pg/m L,IL-6分别为(223.1±20.6)pg/m L和(423.6±35.4)pg/m L,观察组治疗后Th2细胞因子明显低于对照组(t值为1.977、42.956,P值为0.025、0.000)。结论利福喷丁治疗肺结核的效果更高,有助于改善肺结核患者的Th1和Th2细胞因子水平,有较高的推广价值。Objective To analyze the effect of rifapentine on Th1/Th2 cytokines in patients with pulmonary tuberculosis.Methods 154 patients with tuberculosis treated in our hospital from March 2019 to July 2020 were selected as subjects,and were divided into observation group and control group(77 cases in all).The control group received conventional chemotherapy,while the observation group received the same chemotherapy regimen,but the difference was that rifamentine was used instead of rifampicin.The efficacy of the two groups and the changes of Th1 cytokines(interleukin 2-IL-2,interferonγ-INF-γ)and Th2 cytokines(interleukin 4-IL-4,interleukin 6-IL-6)before and after treatment were compared.Results The total effective rate of observation group and control group was 90.9%and 77.9%,respectively,with statistical significance(χ^(2) value was 4.936,P value was 0.026).Before treatment,IL-2 and INF-γwere(20.2±4.2)pg/mL and(20.1±4.3)pg/mL,respectively,in observation group and control group,and INF-γwere(14.9±2.3)pg/mL and(15.0±2.4)pg/mL,respectively.There was no significant difference in Th1 cytokine index between the two groups before treatment(t value was 0.146,0.264,P value was 0.442,0.396).IL-2 in the observation group and the control group after treatment was(14.6±2.2)pg/mL and(17.3±3.5)pg/mL,respectively.INF-γwas(11.0±1.1)pg/mL and(13.5±1.7)pg/mL,respectively,and Th1 cytokine in the observation group was significantly lower than that in the control group after treatment(t value was 5.731,10.834,P value was 0.000,0.000).Before treatment,IL-4 in the observation group and control group were(19.8±4.2)pg/mL and(20.1±4.0)pg/mL,and IL-6 were(952.6±112.3)pg/mL and(961.5±115.4)pg/mL,respectively.There was no significant difference in Th2 cytokine indexes between the two groups before treatment(t values were 0.449 and 0.485,P values were 0.327 and 0.314).IL-4 in the observation group and the control group after treatment was(18.6±1.5)pg/mL and(19.2±2.2)pg/mL,respectively.IL-6 levels were(223.1±20.6)pg/mL and(423.6
