米非司酮联合地屈孕酮对子宫内膜异位症Kupperman评分及血清VEGF,MMP-9的影响

摘　　要：目的 探讨米非司酮联合地屈孕酮对子宫内膜异位症患者Kupperman评分及血清血管内皮生长因子(VEGF)、基质金属蛋白酶-9(MMP-9)的影响。方法 选取我院2019年3月至2021年3月收治的子宫内膜异位症患者102例作为研究对象,随机分为对照组与观察组各51例,对照组采取地屈孕酮治疗,观察组在对照组基础上联合米非司酮治疗,比较两组患者治疗效果、治疗前后Kupperman评分、血清VEGF与MMP-9水平、血清激素孕酮(P)水平、黄体生成素(LH)、卵泡刺激激素(FSH)变化以及药物不良反应情况。结果 观察组总有效率高于对照组,差异具有统计学意义(P<0.05)。治疗后比较观察组Kupperman评分低于对照组,差异具有统计学意义(P<0.05)。治疗后比较观察组血清VEGF、MMP-9水平低于对照组,差异具有统计学意义(P<0.05)。治疗后观察组P、LH、FSH的血清激素水平低于对照组,差异具有统计学意义(P<0.05)。观察组总的不良反应发生率与对照组比较,差异不显著无统计学意义(P>0.05)。结论 米非司酮联合地屈孕酮对子宫内膜异位症患者治疗效果显著,让低雌激素造成的异常状况不明显,使血清VEGF、MMP-9的水平降低到正常范围,其不良反应发生少,较为安全,可在临床上使用。Objective To investigate the effects of mifepristone combined with didroxyprogesterone on Kupperman score,serum vascular endothelial growth factor(VEGF)and matrix metalloproteinase-9(MMP-9)in patients with endometriosis.Methods 102 patients with endometriosis admitted to our hospital from March 2019 to March 2021 were selected as the research object,and randomly divided into control group and observation group with 51 cases each.The control group was treated with didroxyprogesterone.The observation group was treated with mifepristone on the basis of the control group.The treatment effect,Kupperman score,serum VEGF and MMP-9 levels,serum progesterone(P)levels Changes of luteinizing hormone(LH),follicle stimulating hormone(FSH)and adverse drug reactions.Results The effective rate of the observation group was higher than that of the control group(P<0.05).After treatment,the Kupperman score in the observation group was lower than that in the control group(P<0.05).After treatment,the serum VEGF and MMP-9 levels in the observation group were lower than those in the control group(P<0.05).After treatment,the serum hormone levels of P,LH and FSH in the observation group were lower than those in the control group(P<0.05).There was no significant difference in the incidence rate of adverse reactions between the observation group and the control group(P>0.05).Conclusion Mifepristone combined with dydrogesterone is effective in the treatment of patients with endometriosis,so that the abnormal condition caused by low estrogen is not obvious,and the levels of serum VEGF and MMP-9 are reduced to the normal range.It has less adverse reactions,is relatively safe,and can be used clinically.
