摘  要:目的:探讨有效原子序数(Zeff)值在肺动脉栓塞(PE)诊断中的价值。方法:收集临床怀疑PE的308名受检者,经IQon光谱CT检查,重建后得到碘密度图及Zeff图,分析不同部位、不同类型肺动脉栓子对应肺灌注缺损区与非灌注缺损区的碘密度值、Zeff值差异以及其诊断效能。结果:筛出PE患者72例,检出473个栓子,肺动脉栓子供血区中发现灌注缺损区418处,非灌注缺损区55处。不同位置栓子供血区灌注缺损组碘密度值、Zeff值均较非灌注缺损组低,差异有统计学意义(均P<0.01);附壁锐角型、附壁钝角型、中心型以及全部类型的栓子供血区灌注缺损组碘密度值、Zeff值低于非灌注缺损组,差异有统计学意义(均P<0.001)。碘密度值、Zeff值预测灌注缺损的受试者工作特征曲线下面积均为0.952,切点值分别为1.02 mg/mL、9.59。结论:IQon光谱CT的Zeff值可以有效预测PE的肺灌注缺损。Purpose:To exploring the value of Z Effective(Zeff)value in the diagnosis of pulmonary embolism(PE).Methods:Three hundred and eight clinically suspected PE patients were performed IQon spectral CT examination,and iodine density maps and Zeff maps were reconstructed.The differences in iodine density values and Zeff values between perfusion and non-perfusion areas corresponding to different locations and types of pulmonary arterial embolus were analyzed,as well as their diagnostic efficiency.Results:Seventy-two cases of PE patients were screened out,and 473 emboli were detected.Among the pulmonary artery emboli supply areas,418 perfusion defect areas and 55 non-perfusion defect areas were found.The iodine density value and Zeff value of the perfusion defect group in different locations of thrombus blood supply areas were lower than those of the non-perfusion defect group,and the differences were with statistical significance(P<0.01);The iodine density value and Zeff value of the perfusion defect group in the thrombus blood supply area of acute angle type,obtuse angle type,central type,and all types were lower than those of the non-perfusion defect group,and the differences were with statistical significance(P<0.001).The area under the receiver operating characteristic curve of iodine density value and Zeff value for predicting perfusion defects was 0.952 and 0.952,respectively,with cut-off point values of 1.02 mg/mL and 9.59,respectively.Conclusions:The Zeff value of IQon spectral CT can be used to effectively predict pulmonary perfusion defects in PE.
