摘　　要：目的:探索具有不良孕产史的子宫腺肌病患者行磁共振引导聚焦超声手术(MRgFUS)治疗后对生育妊娠的影响。方法:回顾性研究分析自2019年6月1日至2023年6月1日在我院接受MRgFUS治疗的20例具有不良孕产史的子宫腺肌病患者临床资料,分析统计患者的基本信息、病灶影像学特征、消融非灌注体积比率、术后妊娠情况等,探讨MRgFUS术后对生育情况的影响。结果:20例行MRgFUS治疗的子宫肌腺病患者,年龄28~47岁,平均(35.1±5.7)岁,其中治疗前不孕症9例,自然流产或人工流产6例,辅助生殖失败5例。治疗后20例患者中10例患者共妊娠11次,3例为通过体外受精-胚胎移植(IVF-ET)技术妊娠,8例为自然妊娠,总妊娠率为55%,活产率为45%(9/20)。结论:对于具有不良孕产史的子宫腺肌病患者,MRgFUS治疗后对生育妊娠有改善作用。Purpose:To investigate the influencing factors of pregnancy outcomes in patients with adenomyosis and infertility treated by magnetic resonance-guided focused ultrasound(MgFUS).Methods:A retrospective study was conducted to analyze the clinical data of 20 patients with adenomyosis complicated by infertility and adverse pregnancy history who received MRgFUS.The patient's basic information,maternal history,adenomyosis characteristics,tumor ablation rate,postoperative pregnancy interval,and pregnancy status were compared and analyzed to explore the factors affecting pregnancy after MRgFUS treatment.Results:The 20 patients who underwent MRgFUS for adenomyosis were aged 28-47 years,with an average age of(35.1±5.7)years,including 9 patients complicated by infertility,6 by abortion,5 by failure assisted reproduction.After treatment,10 of the 20 patients had a total of 11 pregnancies,3 were in vitro fertilization-embryo transfer(IVF-ET)assisted pregnancy,8 were spontaneous pregnancy.The pregnancy rate was 55%,of which the live birth rate was 45%(9/20).Conclusion:For patients with adenomyosis who have a history of adverse pregnancy and childbirth,MRgFUS treatment has an improvement effect on fertility.
