钆塞酸二钠MR增强检查后对MRCP图像质量的影响分析

摘  要:目的:研究钆塞酸二钠(Gd-EOB-DTPA)增强后对磁共振胰胆管成像(MRCP)检查图像质量的影响。方法:48例受检者Gd-EOB-DTPA增强前后各进行1次MRCP检查,采用Syngo软件测量增强前后MRCP图像的背景信号强度并进行配对样本t检验,由2名医师对增强前后MRCP图像分别进行胰胆管可视化评分并进行配对样本t检验。结果:48例Gd-EOB-DTPA增强后MRCP图像背景信号强度及肝内、外胆管可视化评分均较增强前明显降低,差异均有统计学意义(P<0.05),且胆管可视化评分随检查时间的延长而逐渐降低,但胰管可视化评分在增强前后差异无统计学意义(P>0.05)。结论:Gd-EOB-DTPA增强后进行MRCP检查能抑制图像的背景信号,但使胆管的可视化程度降低,由此提示选用Gd-EOB-DTPA作对比剂时,MRCP检查必须在增强前进行。Purpose:To study the effect of gadolinium-ethoxybenzyl-diethylenetriamine pentaacetic acid(Gd-EOB-DTPA)on the image quality of magnetic resonance cholangiopancreatography(MRCP).Methods:MRCP was performed in 48 subjects before and after Gd-EOB-DTPA enhancement,Syngo software was used to measure the background signal intensity of MRCP images before and after enhancement,and paired-sample t-test was performed.Two physicians visually scored the pancreaticobiliary duct of MRCP images before and after enhancement and performed paired-sample t-test.Results:Both the background signal intensity and visual scores of intrahepatic and extrahepatic biliary ducts on MRCP images of 48 subjects after Gd-EOB-DTPA enhancement were lower than those before enhancement,with statistically significant differences(all P<0.05).The visual scores of bile ducts gradually decreased with the extension of examination time,but there was no statistically significant difference in visual score of pancreatic duct(P>0.05).Conclusion:The background signal is significantly suppressed in MRCP after the use of Gd-EOB-DTPA,while the visualization of biliary duct is reduced.It is suggested that,MRCP examination must be performed before enhancement when using Gd-EOB-DTPA as a contrast agent.
