银川市20593例新生儿先天性心脏病筛查及随访结果分析

摘　　要：目的 对银川市分娩的新生儿进行先天性心脏病(CHD)筛查及随访。方法 选取2019年8月—2021年8月在银川市6家助产医院分娩的20 593例活产新生儿为研究对象,通过不同的筛查方式筛选可疑CHD患儿,并利用超声心动图确诊CHD的类型,将确诊为CHD的新生儿纳入互联网随访系统并定期随访。结果 20 593例活产新生儿中,确诊CHD患儿472例,发病率为2.29%。在确诊的472例CHD患儿中,前5位CHD类型分别是动脉导管未闭(PDA)(286例,60.59%)、房间隔缺损(ASD)(77例,16.32%)、ASD合并PDA(33例,6.99%)、室间隔缺损(VSD)(30例,6.36%)、VSD合并ASD(25例,5.30%)。在6~12月龄时共随访460例(97.46%),其中自然闭合397例(84.11%),手术干预15例(3.18%),死亡3例(0.64%),未愈合45例(9.53%)仍在随访中,失访12例(2.54%)。结论 对CHD患儿进行互联网线上联合线下随访能提高随访率,有助于改善患儿远期生存质量。Objective To screen and follow up congenital heart disease(CHD)of neonates in Yinchuan City.Methods A total of 20593 live neonates born in six midwifery hospitals in Yinchuan City from August 2019 to August 2021 were selected as the research objects.The children suspected of CHD were screened by different screening methods,and echocardiography was used to determine the types of CHD.The neonates diagnosed as CHD were included in Internet follow-up system and regularly followed up.Results Among 20593 live neonates,472 neonates were diagnosed as CHD,with an incidence rate of 2.29%.Among 472 neonates with CHD,the top five types were patent ductus arteriosus(PDA,286 cases,60.59%),atrial septal defect(ASD,77 cases,16.32%),ASD combined with PDA(33 cases,6.99%),ventricular septal defect(VSD,30 cases,6.36%),VSD combined with ASD(25 cases,5.30%).A total of 460 cases(97.46%)were followed up at the age of 6-12 months,of which 397 cases(84.11%)were spontaneously closed,surgical intervention was performed in 15 cases(3.18%),3 cases(0.64%)died,45 cases(9.53%)who did not heal were still under follow-up,12 cases(2.54%)were lost to follow-up.Conclusion Online combined with offline follow-up method can increrase the follow-up rate of neonates with CHD and improve the long-term quality of life.
