地屈孕酮和醋酸甲羟孕酮治疗围绝经期异常子宫出血的效果比较

摘　　要：目的 比较地屈孕酮和醋酸甲羟孕酮治疗围绝经期异常子宫出血的效果。方法 选取2020年2月—2021年8月在瑞安市人民医院接受治疗的200例围绝经期异常子宫出血患者,依据就诊时间分为观察组(使用醋酸甲羟孕酮治疗)与对照组(使用地屈孕酮治疗),每组各100例。比较两组患者治疗3个月后的疗效、临床症状改善情况以及不良反应。结果 观察组患者治疗总有效率为98.00%,高于对照组的91.00%,差异有统计学意义(χ^(2)=4.713,P<0.05)。治疗后,观察组患者腰骶酸痛、头晕耳鸣临床症状评分均低于对照组(均P<0.05),两组手足心热临床症状评分比较,差异无统计学意义(P>0.05)。观察组患者不良反应发生率(3.00%)低于对照组(10.00%),差异有统计学意义(χ^(2)=4.031,P<0.05)。结论 醋酸甲羟孕酮与地屈孕酮治疗围绝经期异常子宫出血的效果均十分显著,醋酸甲羟孕酮的治疗效果优于地屈孕酮,总有效率更高。两种药物对手足心热的调节效果无差异,但对其他指标的调节效果醋酸甲羟孕酮好于地屈孕酮,醋酸甲羟孕酮的不良反应更少,值得推广。Objective To compare the efficacies of dydrogesterone and medroxyprogesterone acetate in treatment of abnormal uterine bleeding during perimenopause.Methods A total of 200 patients with abnormal uterine bleeding during perimenopause who were treated in Ruian People's Hospital from February 2020 to August 2021 were selected and divided into observation group(100 patients treated with medroxyprogesterone acetate)and control group(100 patients treated with dydrogesterone)according to the visit time.The treatment effects,improvements of clinical symptoms,and adverse reactions after 3 months in the two groups were compared.Results The total effective rate of treatment in observation group was 98.00%,which was higher than that in control group(91.00%),there was statistically significant difference(χ^(2)=4.713,P<0.05).After treatment,the clinical symptom scores of lumbosacral pain,dizziness,and tinnitus in observation group were statistically significantly lower than those in control group(P<0.05),there was no statistically significant difference in clinical symptom score of feverishness in palms and soles between the two groups(P>0.05).The incidence rate of adverse reactions in observation group was 3.00%,which was lower than that in control group(10.00%),there was statistically significant difference(χ^(2)=4.031,P<0.05).Conclusion The effects of medroxyprogesterone acetate and dydrogesterone in treatment of abnormal uterine bleeding during perimenopause are significant,the curative effect of medroxyprogesterone acetate is superior to dydrogesterone,the total effective rate is higher.The regulating effects of medroxyprogesterone acetate and dydrogesterone for feverishness in palms and soles are undifferentiated,the regulating effect of medroxyprogesterone acetate for the other indexes is superior to dydrogesterone,medroxyprogesterone acetate has fewer adverse reactions,which is worth promoting.
