摘　　要：持续性姿势性知觉性头晕(persistent postural perceptual dizziness,PPPD)近年来就诊病例呈上升趋势。该疾病于2017年才被《国际前庭疾病分类》进行明确的定义,因此对于其发病机制的研究尚不明确,其发作诱因较为复杂,并且该疾病对病人的精神、心理影响颇深,病人日常生活遭受限制,且西药治疗方案有限。从症候上讲,PPPD属于中医的“眩晕”与“郁病”之合病。王艳昕,教授,安徽中医药大学第一附属医院神经内二科主任,不仅被誉为安徽省名中医,也成立了王艳昕安徽省名中医工作室,是研究团队中的核心成员,王教授也是中国民族医药学会睡眠分会副主任委员、安徽省神经科学学会常务理事,以及中国中药学会脑病分会眩晕学组常委、安徽省中医药学会脑病专业委员会常务委员,20余年来致力于脑病研究,临床经验丰富。王教授总结得出:PPPD当从虚实论治,病机不外乎累及心肝脾,肝失疏泄,脾失建运,心神失养而发此病,治以疏肝解郁,养血安神,化痰健脾,治疗上方以自拟方肝脾调神方,结合虚实进行诊治,多年临床工作中形成了自己独特的经验。作者有幸跟师,现将王师治疗PPPD的临床经验总结如下,以飨同道。The number of patients with persistent postural perceptual dizziness(PPPD)has been increasing in re-cent years.The disease was only clearly defined by the International Classification of Vestibular Diseases in 2017,so the research on its pathogenesis is still unclear.The causes of its attacks are relatively complex,and the disease has a profound impact on the patient’s mental and psychological state.The patient’s daily life is restricted,and the treat-ment options for Western medicine are limited.In terms of symptoms,PPPD belongs to the combination of“vertigo”and“depression”in traditional Chinese medicine.Professor WANG Yanxin,director of the Second Department of Neurology of the First Affiliated Hospital of Anhui University of Traditional Chinese Medicine,is not only known as a famous Chinese medicine doctor in Anhui Province,but also established the WANG Yanxin Anhui Famous Chinese Medicine Studio.He is a core member of the research team.Professor WANG is also the deputy director of the Sleep Branch of the Chinese Society of Ethnomedicine,the executive director of the Anhui Neuroscience Society,and the standing member of the Vertigo Group of the Encephalopathy Branch of the Chinese Society of Traditional Chinese Medicine,and the standing member of the Encephalopathy Professional Committee of the Anhui Society of Traditional Chinese Medicine.He has been committed to brain disease research for more than 20 years and has rich clinical ex-perience.Professor WANG concluded that PPPD should be treated from the perspective of deficiency and excess.The pathogenesis is nothing more than the involvement of the heart,liver and spleen.The liver fails to dredge and release,the spleen fails to build and transport,and the mind fails to nourish.This disease is caused by soothing the liver and relieving depression,nourishing blood and calming the mind,resolving phlegm and strengthening the spleen.The above treatment is based on the self-made liver and spleen regulating mind prescription,combined with deficiency and
