当归补血汤合四物汤对股骨骨折术后切口愈合的疗效观察

摘　　要：目的:观察当归补血汤合四物汤对股骨骨折术后切口愈合的临床疗效。方法:将符合标准的120例股骨骨折术后患者随机分组,每组各60例。对照组给予常规术后预防并发症处理,治疗组在此基础上给予当归补血汤合四物汤治疗。比较2组患者术后1、3、7 d视觉疼痛模拟评分(VAS),红细胞计数(RBC),血红蛋白(Hb)以及血清中炎性指标肿瘤坏死因子-α(TNF-α)、白细胞介素1β(IL-1β)和白细胞介素6(IL-6)等表达水平,记录患者术后患肢消肿时间、住院时间、切口愈合时间以及不良反应发生情况。结果:疗程结束后2组患者疼痛VAS评分降低明显,第3天、7天 VAS评分均低于对照组(P<0.05);治疗后2组患者RBC与Hb水平均高于治疗前,血清中炎性因子TNF-α、IL-1β与IL-6表达则被抑制,治疗组效果优于对照组(P<0.05);治疗组患者术后消肿时间、住院时间与切口愈合时间均短于对照组患者(P<0.05);比较2组药物不良反应发生率(5%vs13.33%),治疗组发生率明显低于对照组(P<0.05)。结论:当归补血汤合四物汤能够缓解股骨骨折术后疼痛程度,上调血液中RBC和Hb水平,同时降低炎性因子表达和不良反应发生率,有效促进切口愈合,值得临床推广。Objective:To observe the clinical efficacy of Danggui Buxue Tang combined with Siwu Tang on wound healing after femoral fracture surgery.Methods:A total of 120 patients who met the criteria after femoral fracture sur-gery were randomly divided into two groups,60 cases in each group.The control group was given routine postoperative complication prevention treatment,and the treatment group was given Danggui Buxue Tang combined with Siwu Tang on this basis.The visual analogue scale(VAS),red blood cell count(RBC),hemoglobin(Hb)and serum inflammatory markers tumor necrosis factor-α(TNF-α),interleukin 1β(IL-1β)and interleukin 6(IL-6)were compared between the two groups 1,3,and 7 days after surgery.The swelling time of the affected limb,hospitalization time,wound heal-ing time and adverse reactions were recorded.Results:After the treatment,the pain VAS scores of the two groups of patients decreased significantly,and the VAS scores on the 3rd and 7th days were lower than those of the control group(P<0.05);after treatment,the RBC and Hb levels of the two groups of patients were higher than before treatment,and the expression of inflammatory factors TNF-α,IL-1βand IL-6 in serum was inhibited,and the effect of the treat-ment group was better than that of the control group(P<0.05);the postoperative swelling time,hospitalization time and wound healing time of the treatment group were shorter than those of the control group(P<0.05);the incidence of ad-verse drug reactions in the two groups(5%vs 13.33%)was significantly lower in the treatment group than in the control group(P<0.05).Conclusion:Danggui Buxue Decoction combined with Siwu Decoction can relieve the pain degree after femoral fracture surgery,increase the levels of RBC and Hb in the blood,and reduce the expression of inflammatory factors and the incidence of adverse reactions,effectively promote wound healing,and is worthy of clinical promotion.
