基于“阳化气,阴成形”理论探讨雌孕激素在银屑病中的作用

摘　　要：银屑病是一种慢性炎症性皮肤病,其发病机制与内分泌紊乱、免疫功能失调等因素有关。妊娠期银屑病患者有明显好转或自愈倾向,研究表明,妊娠期银屑病患者55%有所改善,雌二醇是最强效的天然雌激素,雌激素水平的增加,尤其是相对于孕酮的雌激素水平的增加,与银屑病的改善相关。雌孕激素作为一种重要的激素调节物质,具有调节内分泌、抑制炎症反应、促进皮肤修复等多种生物活性,与“阳化气,阴成形”理论中的阴阳平衡原则相契合。阳气主疏散、化生,阴液主滋养、成形,“阳化气,阴成形”理论认为,二者相互依存、相互制约,共同维护生物体的生命活动。基于“阳化气,阴成形”理论,银屑病的角质形成细胞过度增生,属于阳气化生不足,阴液成形太过。雌激素属阳,孕激素属阴,雌孕激素协同作用调节阴阳平衡,进而改善银屑病。Psoriasis is a chronic inflammatory skin disease,and its pathogenesis is related to endocrine disorders,immune dysfunction and other factors.The patients with psoriasis during pregnancy have obvious improvement or self-healing tendency.Studies show that 55%of patients with psoriasis during pregnancy have improved,and estradiol is the most potent natural estro-gen.And an increase in estrogen levels,especially relative to progesterone,is associated with the improvement of psoriasis.As an important hormone regulating substance,estrogen and progesterone have many biological activities,such as regulating endocrine secretion,inhibiting inflammatory reaction and promoting skin repair,which is consistent with the principle of balance between Yin and Yang in the theory of“Yang transforming into Qi,while Yin constituting form”.Yangqi is responsible for dispersing and transforming,while Yin fluid is responsible for nourishing and shaping.According to the theory of“Yang transforming into Qi,while Yin constituting form”,they are interdependent and mutually restrictive,jointly maintaining the life activities.Based on the theory of“Yang transforming into Qi,while Yin constituting form”,the keratinocytes of psoriasis are excessively proliferated,which belongs to the deficiency of Yang transforming,and the Yin forming is too much.Estrogen belongs to Yang,progesterone belongs to Yin,and estrogen and progesterone cooperate to adjust the balance of Yin and Yang,thus improving psoriasis.
