彩色多普勒超声心动图检查对慢性风湿性心脏病的临床诊断价值研究

摘　　要：目的探究慢性风湿性心脏病应用彩色多普勒超声心动图(CDE)检查的价值。方法选取2021年1月至2023年1月期间周口市中心医院收入的120例慢性风湿性心脏病患者,对其开展CDE检查,并将病理检查作为金标准,进而分析CDE的诊断价值。结果经过对120例研究对象进行病理检查,其中阳性102例,阴性18例。其中CDE诊断真阳性92例、假阳性4例、真阴性14例、假阴性10例。CDE准确度、特异度、敏感度分别为88.33%、77.78%、90.20%。经过对120例研究对象进行病理检查,其中二尖瓣钙化44例、二尖瓣粘连30例、混合型瓣28例,共计102例。CDE检查结果显示,二尖瓣钙化38例、二尖瓣粘连28例、混合型瓣26例,共计92例,检出率情况为90.20%。CDE诊断的95%CI 0.673~1.000,曲线下面积为0.839。结论慢性风湿性心脏病患者的临床诊断中,应用CDE检查的价值较为理想,诊断效能较佳,可为其后续治疗提供可靠依据,具有临床应用意义。Objective To explore the value of Color Doppler Echocardiography(CDE)in the diagnosis of chronic rheumatic heart disease.Methods A total of 120 patients with chronic rheumatic heart disease admitted to Zhoukou Central Hospital from January 2021 to January 2023 were selected,and CDE examinations were performed.Pathological examination results were used as the gold standard to analyze the diagnostic value of CDE.Results Pathological examination of the 120 subjects revealed 102 positive cases and 18 negative cases.CDE diagnosed 92 true positive cases,4 false positive cases,14 true negative cases,and 10 false negative cases.The accuracy,specificity,and sensitivity of CDE were 88.33%,77.78%,and 90.20%,respectively.Among the 120 subjects,there were 44 cases of mitral valve calcification,30 cases of mitral valve adhesion,and 28 mixed valve cases,totaling 102 cases.The CDE examination showed 38 cases of mitral valve calcification,28 cases of mitral valve adhesion,and 26 mixed valve cases,totaling 92 cases,resulting in a detection rate of 90.20%.The diagnostic 95%CI for CDE was 0.673~1.000,and the area under the curve was 0.839.Conclusion The application of CDE in the clinical diagnosis of chronic rheumatic heart disease is of ideal value,showing good diagnostic efficacy and providing reliable evidence for subsequent treatment,which has significant clinical implications.
