摘 要:目的探讨重症监护室(ICU)休克患者采用气道精细化护理联合风险管理对呼吸机相关性肺炎(VAP)发生率的影响。方法选取2020年1月至2022年11月郑州市中医院收治的68例ICU休克患者,以随机数字表法分为干预组(n=34,气道精细化护理联合风险管理)与对照组(n=34,常规护理),比较两组住ICU时间、住院时间、VAP发生率、急性生理学和慢性健康状况评价Ⅱ(APACHEⅡ)评分、快速急诊内科评分(REMS)。结果干预组住ICU时间及总住院时间均短于对照组,VAP发生率低于对照组,差异有统计学意义(P<0.05);两组干预后APACHEⅡ评分均降低,且干预组低于对照组,差异有统计学意义(P<0.05);两组干预后REMS评分均降低,且干预组低于对照组,差异有统计学意义(P<0.05)。结论气道精细化护理联合风险管理应用于ICU休克患者,可缩短住院时间,减少VAP发生,改善生理状况。Objective To explore the effect of fine airway nursing combined with risk management on the incidence of ventilator-associated pneumonia(VAP)in shock patients in the Intensive Care Unit(ICU).Methods We selected 68 ICU shock patients admitted to Zhengzhou Traditional Chinese Medicine Hospital from January 2020 to November 2022 and divided them into an intervention group(n=34,fine airway nursing combined with risk management)and a control group(n=34,routine nursing)using a random number table method.We compared the duration of ICU stay,total hospitalization time,VAP incidence,Acute Physiology and Chronic Health EvaluationⅡ(APACHEⅡ)scores,and Rapid Emergency Medicine Score(REMS).Results The intervention group had shorter ICU stay and total hospitalization time compared to the control group,and the incidence of VAP was lower in the intervention group,showing statistical significance(P<0.05).Both groups showed a reduction in APACHEⅡscores post-intervention,with the intervention group scoring lower than the control group,demonstrating statistical significance(P<0.05).Similarly,REMS scores decreased in both groups post-intervention,with the intervention group lower than the control group,also indicating statistical significance(P<0.05).Conclusion The application of fine airway nursing combined with risk management in ICU shock patients can shorten hospitalization time,reduce the incidence of VAP,and improve physiological status.
