摘  要:目的采用孟德尔随机化方法研究233种循环代谢生物标志物与前列腺炎、前列腺增生、前列腺原位癌之间的因果关系。方法暴露与结局的数据均来自公开发表的大规模全基因组关联研究(GWAS),从GWAS数据中选择与暴露因素具有强相关性的单核苷酸多态性(SNP)作为工具变量(IV)。采用逆方差加权法、加权中位数法、MR-Egger、加权模式简单模式研究暴露与结局之间的因果关系,并进行敏感性分析验证结果的稳健性。结果小极低密度脂蛋白(VLDL)中磷脂与总脂质的比值是前列腺炎的保护因素,亮氨酸水平是前列腺炎危险因素。低密度脂蛋白(LDL)颗粒的平均直径和小VLDL中游离胆固醇与总脂质的比值是前列腺增生的保护因素。小高密度脂蛋白(HDL)中的总脂质含量和小HDL颗粒浓度是前列腺原位癌的保护因素。结论循环代谢生物标志物与前列腺疾病密切相关。Objective To investigate the causal relationships between 233 circulating metabolic biomarkers and prostatitis,benign prostatic hyperplasia,and prostate cancer using Mendelian randomization methods.Methods Exposure and outcome data were derived from publicly available genome-wide association studies(GWAS).Single nucleotide polymorphisms(SNPs)strongly associated with the exposure factors were selected from GWAS data as instrumental variables(IVs).The causal relationships between exposure and outcome were examined using inverse variance weighted,weighted median,MR-Egger,and weighted mode methods in a simple mode.Sensitivity analyses were conducted to verify the robustness of the results.Results The ratio of phospholipids to total lipids in Very Low-Density Lipoprotein(VLDL)was identified as a protective factor for prostatitis,while leucine levels were a risk factor.The average diameter of Low-Density Lipoprotein(LDL)particles and the ratio of free cholesterol to total lipids in small VLDL were protective factors for benign prostatic hyperplasia.Total lipid content in High-Density Lipoprotein(HDL)and small HDL particle concentration were protective factors for prostate cancer in situ.Conclusion Circulating metabolic biomarkers are closely associated with prostate diseases.
