芒针透刺联合推拿对湿热蕴结证痛风性关节炎患者的影响

摘　　要：目的探究芒针透刺联合推拿对湿热蕴结型痛风性关节炎的影响。方法选取2024年6月-2024年9月于宿州市中医院就诊的82例湿热蕴结型痛风性关节炎患者为研究对象,经配对随机分组法分为观察组及对照组,每组各41例,其中对照组采用双氯芬酸钠治疗,观察组在对照组基础上采用芒针透刺联合推拿治疗。于治疗前及治疗4周后比较两组患者中医证候积分、血清疾病相关生化指标[Nod样受体蛋白3炎性体(NLRP3)、白细胞介素-1β(IL-1β)、尿酸(SUA)]、关节肿胀程度、疼痛程度[疼痛数字评分法(NRS)]的差异。结果治疗4周后观察组中医证候积分、血清相关生化指标(NLRP3、IL-1β、SUA)、关节肿胀程度、NRS评分均明显低于对照组(P<0.05)。结论芒针透刺联合推拿能抑制湿热蕴结型痛风性关节炎患者的炎症反应,降低其疼痛程度,改善关节肿胀程度,减轻其临床症状,对湿热蕴结型痛风性关节炎患者具有积极影响。Objective To explore the influence of awn needle penetration acupuncture combined with massage on gouty arthritis of damp-heat accumulation syndrome.Methods 82 patients with gouty arthritis of damp-heat accumulation syndrome who were treated in Suzhou Hospital of Traditional Chinese Medicine from June 2024 to September 2024 were selected as the research subjects,and were divided into observation group and control group by paired random grouping method,with 41 cases in each group.The control group was treated with diclofenac sodium,while the observation group was treated with awn needle penetration acupuncture combined with massage on the basis of the control group.The TCM syndromes scores,serum disease-related biochemical indicators[Nod-like receptor protein 3 inflammasome(NLRP3),interleukin-1β(IL-1β),serum uric acid(SUA)],joint swelling degree and pain degree[Numerical Rating Scale(NRS)]were compared between groups before treatment and after 4 weeks of treatment.Results After 4 weeks of treatment,the TCM syndromes scores,serum disease-related biochemical indicators(NLRP3,IL-1β,SUA),joint swelling degree and NRS score in observation group were significantly lower than those in control group(P<0.05).Conclusion Awn needle penetration acupuncture combined with massage can inhibit the inflammatory response in patients with gouty arthritis of damp-heat accumulation syndrome,reduce the pain degree,improve the degree of joint swelling,and relieve the clinical symptoms,and it has a positive impact on patients with gouty arthritis of damp-heat accumulation syndrome.
